摘 要:目的 探讨伴神经内分泌 (NE)细胞分化的乳腺癌的恶性程度及生物学行为。方法 用免疫组化法(L SAB)对 131例乳腺癌进行铬粒素 (chromogranin)、c- erb B- 2、PS2及前列腺特异抗原 (PSA)表达检测 ,以经典的Feulgen染色法及图像分析技术对 81例乳腺癌进行 DNA定量分析。结果 NE细胞阳性乳腺癌 c- erb B- 2阳性表达率为 37.5 %及表达强度 (++~ +++) 33.3%均明显低于 NE细胞阴性肿瘤的 6 2 .6 %和 6 8.7% (P<0 .0 5 ,P<0 .0 5 ) ;NE细胞阳性乳腺癌 PS2、PSA表达率分别为 72 .2 %和 5 5 .0 % ,均显著高于 NE细胞阴性肿瘤的 45 .0 %与16 .4% (P<0 .0 5 ,P<0 .0 5 ) ;NE细胞 (+)组 DNA指数、DNA主干系峰值、>5 C异倍体细胞数以及异倍体率均显著高于 NE细胞 (++)乳腺癌组 (P<0 .0 5 ) ,NE细胞 (++)乳腺癌组积分吸光度、DNA指数、DNA主干系峰值、>5 C异倍体细胞数以及异倍体率均明显低于 NE细胞阴性组 (P<0 .0 5 ) ;在 ~ 级乳腺癌中 ,NE细胞阳性组的积分吸光度、DNA指数、DNA主干峰值、>5 C异倍体细胞数以及异倍体率均显著低于 NE细胞阴性组 (P<0 .0 1)。结论 伴神经内分泌分化的乳腺癌恶性程度较低 ,预后较好。Objective To investigate the malignant degree and biological behavior of breast carcinomas with neuroendocrine (NE) cells differentiation. Methods c-erbB-2,PS2,the prostate-specific antigen (PSA) expression in tissue specimens of 131 cases of breast carcinomas were detected by immunohistochemistry (LSAB method) and image analysis for DNA content was performed with Feulgen staining in 81 cases of breast carcinomas. Results The positive rate of c-erbB-2 was 37.5% in NE positive cases and 33.3% showed an intensity of ++~+++,significantly lower than those in NE negativecases ( P <0.05).The positive rates of PS2 and PSA were 72.2% and 55.0% in NE positive cases respectively,significantly higer than those in NE negaive cases.The DNA index,DNA stemlines peak,>5C aneuploidy cells and the rate of aneuploidy cells in the NE(+) breast carcinoma cases were all higer than those in the NE(++) breast carcinoma cases ( P <0.05).The integral optic density,DNA index,DNA stemlines peak,>5C aneuploidy cells and the rate of aneuploidy cells in the NE(++) breast carcinoma cases were all lower than those in the NE(-) breast carcinoma cases ( P <0.05).Among grad Ⅰ and Ⅱ breast carcinomas,the integral optic density,DNA index,DNA stemlines peak,>5C aneuploidy cells and the rate of aneuploidy cells in the NE positive breast carcinoma cases were all lower than those in the NE negative breast carcinoma cases ( P <0.01). Conclusion The NE positive breast carcinoma may have better prognosis due to its lower malignancy.\;
