Ki-67及p53蛋白在口腔鳞状上皮肿瘤中的表达及意义

摘　　要：目的 :探讨口腔鳞状上皮肿瘤Ki 6 7、p5 3蛋白阳性表达与肿瘤发生、发展的关系。方法 :应用Ki 6 7、p5 3抗体对口腔鳞癌、疣状癌、乳头状瘤及正常粘膜上皮进行S P法免疫组化检测 ,对阳性反应切片进行光镜下观察 ,计数阳性细胞百分比 ,各组肿瘤间进行比较。结果 :鳞癌各例Ki 6 7、p5 3均呈阳性表达 ,疣状癌Ki 6 7、p5 3(除 1例 )均呈阳性表达 ,乳头状瘤Ki 6 7均呈阳性表达 ,p5 3阴性表达。正常粘膜上皮Ki 6 7、p5 3均呈阴性表达。Ki 6 7、p5 3阳性细胞百分比均随肿瘤恶性度增高而增加 ,各组间差异十分显著 (P均 <0 .0 1)。结论 :Ki 6 7,p5 3阳性表达与肿瘤发生、发展密切相关 ,可作为判断肿瘤良恶性及恶性程度的辅助指标。Objective: Our purpose was to investigate the positive expressions of Ki 67 and p53 protein and the relationship with the pathogenesis and progress in oral squamous epithelial tumor. Methods: The oral papillomas, verrucous carcinomas, squamous cell carcinomas, and healthy mucosal epithelium were examined by using S P immunohistochemistric staining with Ki 67 polyclonal antibodies and p53 monoclonal antibody. The cells with positive expression were studied using microscopy and the percent of these cells was counted and compared between groups. Results: The expressions of Ki 67 and p53 were positive in all squamous cell carcinomas. Positive expressions of Ki 67 and p53 were also seen in all but one case of verrucous carcinomas.The expression of Ki 67 in human papillomas was positive but that of p53 was negative . The percent of posotive cells increased with the malignant degree, and the difference between groups was significant (P<0.001, P<0.003). Conclusion: The positive expressions of Ki 67 and p53 play an important role in pathogenesis and progress of tumor. It would be an assistant index in distinguishing the malignant and benign tumor and the malignant degree.
