胆囊癌中CD44v6和基质金属蛋白酶-2的表达及其临床意义

摘　　要：目的 :探讨胆囊癌中粘附分子CD4 4v6、基质金属蛋白酶 2 (matrixmetalloproteinase 2 ,MMP 2 )的表达及临床意义 ,以及二者之间的关系。方法 :采用免疫组织化学方法检测 4 2例胆囊癌、10例慢性结石性胆囊炎组织中CD4 4v6和MMP 2的表达。结果 :①CD4 4v6阳性表达率在胆囊癌组为 6 4 .3% (2 7/ 4 2 ) ,明显高于慢性结石性胆囊炎组 (0 / 10 ) (P <0 .0 0 5 ) ;CD4 4v6表达与胆囊癌的组织类型、病理分级以及转移相关 (P <0 .0 5 ) ;CD4 4v6阳性的胆囊癌病人预后较差。②MMP 2在胆囊癌组织中表达率为 6 1.9% ,明显高于慢性结石性胆囊炎组 (10 % ,P <0 .0 0 5 ) ;MMP 2的表达与胆囊癌细胞的分化、转移和预后有关 (P <0 .0 5 ) ,与组织类型无关。③CD4 4v6和MMP 2在胆囊癌组织中表达呈正相关 (P <0 .0 0 5 )。结论 :CD4 4v6和MMP 2的表达与胆囊癌分化、转移、预后有关 ,在胆囊癌细胞的侵袭和转移中起重要作用。联合检测有利于评估胆囊癌的生物学行为和判断预后。Objective:To study the clinical significance of expression of CD44v6 and MMP 2 in the Gallbladder Carcinoma(GC) and relationship between CD44v6 and MMP 2.Methods:To detect the expression of CD44v6 and MMP 2 in 42 cases of GC and 10 cases of cholecystolithiasis by immunohistochemical techniques.Results:①The expression of CD44v6 in the GC(64.3%,27/42) was significantly higher than that in cholecystolithiasis(0,0/10)(P<0.05). The expression of CD44v6 was correlated with histological grade,histological type and matastasis in GC(P<0.05). There was a significant survival advantage in patients with high expression of CD44v6,as compared with low expression(P<0.05).②The expression rate of MMP 2 was 61.9%(26/42) in GC and signigicantly higher than that in cholecystolithiasis(10%,P<0.005). The expression of MMP 2 was correlated with histologic grade,metastasis and prognosis of GC(P<0.05),and was not correlated with histologic type of GC(P>0.05).③The CD44v6 expression was positive correlation to MMP 2 expression in GC(P<0.05).Conclusion:①The expression of CD44v6 and MMP 2 was correlated with pathological grade,metastasis and prognosis. CD44v6 and MMP 2 play an important role in the invasion and metastasis of the GC.②Examination of expression of these two proteins may be helpful to judge biological behavior and prognosis of GC.
