摘　　要：目的:探讨肺腺鳞癌的CT表现特点及瘤内的腺癌、鳞癌成分含量与CT表现的关系。材料与方法:收集经我院胸外科手术病理证实的15例肺腺鳞癌,男13例,女2例,全部病例均行胸部CT检查。根据病理光镜下腺癌和鳞癌两种成分所占比例分为鳞癌成分占优势组(1组)和腺癌成分占优势组(2组)两组。结果:15例中1组7例,2组8例。①CT表现:中央型3例,表现为肺门肿块伴肺不张;周围型12例,主要征象有分叶征、毛刺征、小泡征、血管集束征及胸膜尾征。②肿瘤内腺、鳞癌成分含量与病变的影像分型、大小及分布有一定相关性,与其他CT表现无明显关系。结论:肺腺鳞癌男性好发,周围型多见;中央型肺腺鳞癌中鳞癌成分占优势,周围型肺腺鳞癌的CT表现与肺腺癌相似。Objective:To study CT features of pulmonary adenosquamous carcinoma(ASC) and its relationship with pathological components.Materials and Methods:Fifteen cases(13 males,2 females) of pulmonary ASC confirmed by surgery and pathology were collected for this study.All cases received chest CT examinations.These cases were divided into two groups according to the proportion of the two malignant components of ASC under microscopic study:the first group in which squamous cell carcinoma was predominant;the second group in which adenocarcinoma was predominant.Results:In the first group contained 7 and the second group contained 8 cases. ①CT manifestations:3 cases were of the central type lung carcinoma and showed hilar mass accompanying atelectasis;12 cases were of the peripheral types, there was maso-like lesion with lobulated or spiculated margin,vesicles within the lesion,vessel convergence sign and pleural tag sign were also common.②The amount of the two malignant cell type in pulmonary ASC was related to tumor image typing,size and distribution;and had no significant relationship with other CT manifestations.Conclusion:The majority of pulmonary ASC occurred in male and of the peripheral type.The central type of pulmonary ASC was predominantly squamous cell carcinoma.CT manifestations of peripheral pulmonary ASC are similar to those of adenocarcinoma.
