我国三城市社区人群开展干预9年脑卒中死亡率的变化

摘　　要：目的探索适合国情的社区人群降低脑卒中发病率和死亡率的有效预防措施。方法在脑卒中高发城市北京、上海、长沙三个市区约 30万人群中开展社区综合性干预研究 ,其中约半数设为对照人群。结果开展加强干预 9年 ,干预社区男性脑卒中死亡率下降了 49.5% ,女性下降了 1 6.0 % ;男女合计年龄调整死亡率下降了 53.6%。同期对照社区男女死亡率也分别下降了 1 7.6%和 34.4%。干预社区 9年平均死亡率为 99.3/ 1 0万 ,对照社区为 1 1 6.1 / 1 0万 ,统计学比较差异有显著性意义 (P<0 .0 1 )。在干预社区 50岁以上高龄组死亡专率逐年下降的同时 ,35～ 45岁组死亡率却显示出上升趋势。结论在人群中开展经常化的健康教育和健康促进活动 。Objective To explore the effective and suitable measures for lowering incidence and mortality of stroke in community po pulation. Methods A community based comprehensive intervention trial was conducted among 300 000 population in three urban communities in Beijing,Shanghai and Changsha.About half population was designed as a control group.Results In the past nine years,the mortality of stroke was decreased by 49.5% in male and 16.0% in female,and the total age adjusted mortality decreased by 53.6% in the intervention community.During the same period,the mortality of stroke in control community also decreased by 17.6% in male and 34.4% in female respectively.The average mortality rates were 99.3/100 000 in intervention population and 116.1/100 000 in control population,which showed a statistically significant difference between the two groups ( P <0 01).It should be noticed that the age specific mortality rate in the age 35~49 group has been increased gradually in recent years.Conclusion An intensive prevention and control on hypertension together with a regular health education and health promotion in community population could evidently reduce the motality of stroke.
