摘 要:目的 探讨粘附分子CD 44v6 ,基质金属蛋白酶 -2 (MMP -2 )在胆囊癌侵袭和转移中的作用。 方法 采用免疫组织化学方法检测了 42例胆囊癌、10例慢性结石性胆囊炎组织中CD 44v6和MMP -2的表达。结果 ( 1)CD44v6阳性表达率在胆囊癌组 ( 6 4.3% ,2 7/42 )明显高于慢性结石性胆囊炎组 ( 0 % ,0 /10 ) (P <0 .0 0 5 ) ;CD 44v6表达强度与胆囊癌的病理学分级相关 (P <0 .0 5 ) ,低分化与中、高分化组差异有显著性 ( P <0 .0 1) ;胆囊癌中转移组阳性率明显高于非转移组 ( P <0 .0 1) ;CD44v6阳性的胆囊癌病人预后较差。 ( 2 )MMP -2在胆囊癌组织中表达率 ( 6 1.9% )明显高于慢性结石性胆囊炎组 ( 10 % ,P <0 .0 0 5 ) ;MMP -2的表达与胆囊癌细胞的分化、转移和预后有关 ( P <0 .0 5 ) ;随着分化程度的恶化 ,其阳性率增高。 ( 3)CD44v6和MMP -2在胆囊癌组织中表达呈正相关(P <0 .0 0 5 )。 结论 CD44v6 ,MMP -2的表达与胆囊癌分化、转移及预后有关 ,在胆囊癌细胞的侵袭和转移中起重要作用。Objective To evaluate the role of CD44v6 and matrix metalloproteinase-2(MMP-2) in the invasion and metastasis of gallbladder carcinoma (GC). Methods Immunohistochemical techniques were adopted to detect the expression of CD44v6 and MMP-2 in 42 cases of GC and 10 cases of cholecystolithiasis. Results (1)The expression of CD44v6 in the GC group (64.3%, 27/42) was significantly higher than that in the cholecystolithiasis group (0%,0/10). The expression of CD44v6 was correlated with GC histologic grade (P< 0.005). The expression of CD44v6 in Grade Ⅲ of GC was significantly higher than that in Grade Ⅰand Grade Ⅱ of GC(P<0.01). The expressions of CD44v6 in GC with histologically positive regional lymph node metastasis and distant metastasis (75.8%) were significantly higher than those without matastasis (22.2%,P<0.01). There was a significant higher survival rate in patients with the high expression of CD44v6 compared with the low expression(P<0.05). (2)The expression rate of MMP-2 in GC group (61.9%,26/42)was significantly higher than that in cholecystolithiasis group (10%,P<0.005). The expression of MMP-2 was correlated with histologic grade, metastasis, and prognosis of GC (P<0.05). With the augmentation of grade, the positively expression rate of CD44v6 was increased.(3)The CD44v6 expression was positively correlated with the MMP-2 expression in GC (P<0.05). Conclusions (1)Overexpressions of CD44v6 and MMP-2 are correlated with pathological grade, metastasis, and prognosis of GC. CD44v6 and MMP-2 play an important role in the invasion and metastasis of the GC. (2)Examination of expression of these two proteins may be helpful to judge the biological behavior and prognosis of GC.
