丽珠胃三联与H2RA或与PPI的四联疗法根除幽门螺杆菌的临床研究

摘　　要：目的 :对丽珠胃三联 +组胺 2受体阻制剂 (H2RA)、LOSEC +丽珠胃三联及丽珠胃三联疗法的疗效、1周疗法与 2周疗法的疗效作一探讨。方法 :经胃镜确诊的HP阳性胃溃疡、十二指肠溃疡及慢性胃炎病人随机分为 6组 ,分别予LOSEC 2 0mg ,2次 /d +丽珠胃三联 4片 2次 /d ,1周和 2周 ;泰胃美 40 0mg ,2次 /d +丽珠胃三联 4片 ,2次 /d 1周和 2周 ;丽珠胃三联 4片 ,2次 /d 1周和 2周治疗。疗程结束 4周后溃疡患者由胃镜复查 ,慢性胃炎病人由C1 3-异素呼气试验复查HP。结果 :1 50例HP阳性病人 ,总根除率为 88% ,各组间无显著性差异 ,P >0 .0 5 ,1周与 2周疗程之间 ,差异也无显著性意义 ,溃疡病治疗的有效率为 98.7% ,治愈率为72 .37% ,其中LOSEC +胃三联组 2周疗程组高于单用胃三联组 (P <0 .0 5) ,且高于泰胃美 +胃三联 1周组 ,P <0 .0 5 ;副作用发生率为 1 4 % ,主要有头昏、恶心、腹泻等 ,病人能耐受 ,依从性较好。结论 :胃三联、胃三联+PPI、胃三联 +泰胃美 1周疗程和 2周疗程均为根治HP的较好治疗方案 ,不依从服药可能是根治失败者的因素之一 ;胃和十二指肠溃疡病的治愈率以LOSEC +胃三联组高于胃三联组 ;药物副作用发生率较高但程度轻 。Objective:To investigate therapeutic effects of lizhuweisanlian (LZWSL) triple therapy\lzwsl+PPI Quadraple therapy\LZWSL+H2RA quadraple therapy,and compare the efficacy of 1 and 2 weeks for eradicating helicobacter pylori (HP). Methods:Patients were randomized to receive the triple or quadraple therapy:LZWSL 4 tablets bid+losec 20mg bid 1 week or 2 weeks;LZWSL 4 tablets bid +tagmet 400mg bid 1 week or 2 weeks;LZWSL 4 tablets did 1 week or 2 weeks.Hp eradication was verified with endoscopy and biopsies for histology in peptic ulcers,and with BC-urea breath test in chronic gastritis at least 4 weeks after completing therapy.Results:The eradication rate was 88% in the 150 patients,there was no significant differance among every group, P >0.05;The efficacy rate for peptic ulcer diseases was 98.7%,the cure rate was 72.37%,and the best therapy method for curing ulcer was LZWSL+losec for 14 days in this study;All patients were well tolerated.Conclusions:LZWSL,LZWSL+PPI and LIWSL+H2RA were better treatment methods for curing HP.It may be a main failure cause in eradicating HP that patients did not comply with the dose;The PPI+LZWSL 2 weeks therapy is better than LZWSL and H2RA+LZWSL 1 week treatment in curing peptic ulcer, P <0.05.
