腹腔与静脉化疗并高频透热治疗中晚期胃癌的临床研究

摘　　要：目的　观察腹腔化疗与静脉化疗联合高频透热治疗中晚期胃癌的近期疗效和毒副反应。方法　随机分为腹腔与静脉化疗并高频透热治疗组 2 2例、单纯腹腔与静脉化疗组 (对照组 ) 2 2例 ,记录观察近期疗效和毒副反应 ,随访患者并计算生存期。按WHO癌症治疗客观疗效评定标准判断疗效 ,毒副反应按WHO1981年统计标准评价。结果　治疗组有效率、平均缓解期、中位生存期、一年生存率分别为 72 .7%、7.6± 1.5个月、11.2± 2 .1个月、6 7.2 % ,对照组分别为36 .3%、4.8± 1.2个月 ,6 .5± 1.8个月、37.6 % ,两组差异有显著性 (P <0 .0 5 )。毒副反应中恶心呕吐、骨髓抑制治疗组分别为 36 .3%、40 .9% ,对照组分别为 2 7.2 %、36 .3% ,两组差异无显著性。治疗组腹胀腹痛 45 .4% ,对照组为 2 7.2 % ,两组差异有显著性 (P <0 .0 5 )。结论　腹腔与静脉化疗并高频透热治疗中晚期胃癌可有效延长生存期 ,并改善其生存质量。Objective To observe the short-term result and side effects of treatment of middle and advanced gastric cancers with chemotherapy combined with high frequency diathermy through abdominal cavity and vein.Methods 44 patients were divided at random into 2 groups;22 cases received chemotherapy combined with hgih frequency diathermy through abdominal cavity and vein as treatment group and the other 22 cases recieved chemotherapy alone.To record and observe the short term results and side effects,they were followed up and the survival time was calculated.The curative effect was determined according to objective effect evaluation standard of WHO cancers treatment,and the side effects were evaluated according to unified standard of WHO in 1981.Results In treatment group, the effective rate,the average response time,median survival time,and the survival rate of 1 year were 72.7%?7.6±1.5 months?11.2±2.1 months,67.2% respectively,and in the control group they were 36.3%?4.8±1.2 months?6.5±1.8 months?7.6% respectively.The difference was significant (P<0.05).In treatment group nausea and vomiting,bone marrow suppression were 36.3%,40.9% individually and in the control group they were 27.2%?36.3%.The difference was not remarkable.In treatment group,abdominal distention and pain was 45.4%,and in control group it was 27.2% The difference was significant.Conclusion The treatment of middle and advanced gastric cancers with chemotherapy combined with high frequency diathermy through abdominal cavity and vein may prolong the survival time and improve their life quality.
