后期加速超分割术前放疗经腹会阴手术治疗局部晚期直肠癌  

Observation of the curative effect from the treatment of locally advanced carcinoma of rectum by means of large-dosage preoperative radiotherapy and joint resection of the abdominal perineum

摘　　要：目的　探讨后期加速超分割术前放疗经腹会阴手术治疗局部晚期直肠癌的疗效。方法　 4 6例局部晚期直肠癌随机分为加速后期超分割术前放疗经腹会阴手术组 2 2例 ,单纯腹会阴手术组 2 4例。术前放疗组先行盆腔前后野常规照射 36Gy/ 4周 ,后行盆腔两侧野超分割照射 ,2 4～ 2 6Gy/ 2周 ,总DT为 6 0～6 2Gy ,休息 4～ 5周后手术。单独手术组术前不放疗。 结果　术前放疗组淋巴结转移率、局部复发率显著低于单纯手术组。手术切除率、3年、5年生存率显著高于单纯手术组。Objective To discuss the long term curative effect resulting from the treatment of local late\|stage carcinoma of the abdominal perineum.Methods We randomly divided 46 patients with local late\|stage carcinoma of rectum into two groups .Group one composed of 22 patients was treated by both large\|dosage radiotherapy and joint resection of the abdominal perineum.Group two of 24 patients was treated merely by resection of the abdominal perineum. The joint resection was performed after 4 to 5 weeks' rest. Group two received Miles' resection after a thorough preoperative preparation .Results The result indicated that the two groups showed significant difference in terms of resection rate (χ 2=5073, P <0.05).The lymoh node metastasis rate of the two groups had no significant difference.The 3\|year and 5\|year relapse rates showed that the two groups had signficant difference,The difference of the 1\|year,3\|year and 5\|year survival rates betweenthe two guoups was significant.Conclusion The result showed that the resection of the abdominal perineum after large\|dosage preoperative radiotherapy was of significance.
