肝门胆管癌基质金属蛋白酶MMP-2及其组织抑制因子TIMP-2表达及意义的研究

摘　　要：目的 :探讨基质金属蛋白酶MMP 2及其组织抑制因子TIMP 2在肝门胆管癌的表达及其与肝门胆管癌浸润、转移及预后间的关系。方法 :采用免疫组化S P法对 78例肝门胆管癌的MMP 2及其组织抑制因子TIMP 2的表达进行了检测。对MMP 2及TIMP 2的表达与肝门胆管癌的组织分化程度、浸润转移及预后之间的关系进行分析。结果 :MMP 2和TIMP 2表达阳性率分别为 72 %和 76 %。MMP 2的阳性表达率有淋巴结浸润的肝门胆管癌高于无淋巴结浸润的肝门胆管癌 ,低分化的肝门胆管癌高于高分化的肝门胆管癌。TIMP 2阳性表达率与以上相反。MMP 2表达与肝门胆管癌术后生存期呈负相关 (r =- 0 .713,P <0 .0 1) ,TIMP 2表达与肝门胆管癌术后生存期呈正相关 (r =0 .6 5 2 ,P <0 .0 1)。MMP 2与TIMP 2具有负相关关系 ,(r =- 0 .70 8,P <0 .0 1)。结论 :MMP 2与TIMP 2是反映肝门胆管癌浸润。Objective:To study the expression of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of metalloproteinase-2 (TIMP-2) in hilar cholangiocarcinoma.To discuss the relationship of MMP-2 and TIMP-2 expression with the metastasis and prognosis of hilar cholangiocarcinoma.Methods:Immunohistochemical S-P method was used in the examination of MMP-2 and TIMP-2 expression in 78 hilar cholangiocarcinoma.The relationships of MMP-2 and TIMP-2 expression with the pathological differentiation,metastasis and prognosis were analyzed.Results:The positive rate of MMP-2 expression was 72% ,and that of TIMP-2 was 76%.According to Ridit analysis,the degree of MMP-2 expression were correlated with the pathologic differentiation and lymph-node metastasis of hilar cholangiocarcinoma.The high expression of MMP-2 existed in the low pathologic differentiation and the high rate of lymph-node metastasis of hilar cholangiocarcinoma.The high expression of TIMP-2 existed in the high pathologic differentiation and the low rate of lymph node metastasis of hilar cholangiocarcinoma. MMP-2 and TIMP-2 expression had a significance correlation(r=0.708,P<0.01).The expression of MMP-2 had significantly correlation with the postoperative survival rate of hilar cholangiocarcinoma (r=-0.713,P<0.01).TIMP-2 expression had a positive correlation with the postoperative survival rate of hilar cholangiocarcinoma(r=0.652,P<0.01).Conclusion:MMP-2 and TIMP-2 can reflect the pathological differentiation,metastases and prognosis of hilar cholangiocarcinoma.
