癫痫的不同临床类型与其脑电图和脑CT检查的结果比较

摘　　要：目的　比较癫痫的不同临床类型与其EEG和脑CT的检查结果。方法　本文根据病史、临床表现、EEG (n=5 5 6)与脑CT或MRI(n =42 7/ 5 5 6)检查结果而确认为癫痫者 ,共 5 5 6例。按其病因分为原发性癫痫、继发性癫痫及混合性癫痫三种临床类型 ,并对其EEG异常率、异常程度、脑波特征以及与脑CT(或MRI)扫描结果进行了比较分析。结果　继发性癫痫的EEG异常率、轻度异常者及出现广泛性、弥散性漫波者所占百分比均居于其它两类型之首 ;原发性癫痫的EEG异常率则低于和接近其它两类型 ,而中、重度异常及痫样放电两者又高于其它两类型 ;混合性癫痫的异常率、异常程度及脑波特征均介于其它两类型之间。本文还表明 ,EEG检查对继发性癫痫有近 90 %的患者是符合脑CT扫描结果的 :对混合性癫痫 (42 7例 )其异常率 (81.76% )却高于脑CT(40 .0 5 % )。本文还评价了EEG检查在定侧、定位方面的重要性。结论　EEG与脑CT(或MRI)两种检查手段各有优劣 ,宜两者兼用 ,若再结合本文结果综合分析 。Objective To compare t he results between and brain CT-test EEG-test in the various clinical types of ep ilepsy.Methods A total of 556 patients with epilepsy who according to history of illness,clinical symptoms,both EEG-test( n =556)and br ain CT-test ( n =427/556)were diagnosed.All of these patients were divied into three clinical types,such as idiopathic epilepsy,secondary epilepsy and mixed e pilepsy(above both in one),and comparison of these results between the proporti ons abnormal EEG,severe degree of EEG,the characters of abnormal brain waves,and the relation of the EEG-test to the brain CT-test.Results ①Secondary epilepsy had the high proportions of abnormal EEG,severe degree of EEG were light,brain waves were broad and spread,all of these were significant high than other types;②Idipathic epilepsy had the proportions of low or approx imate value than other types,but their severe degree of EEG were moderate and he avy,and epilepsy-like spark were high than other types;③Mixed epilepsy had a in termediat value of other types,inclnding 3 items as indicated above.On the other hand,88.89% of EEG-test comform to brain CT-test in the secondary epilepsy;the proportions of abnormal brain CT( 40.05 %) were low than abnormal EEG(81.76%), this finding indicated that EEG-test to be superior brain CT-test alone on mixed epilepsy.An additon,EEG-test may be provide refference when you decided pathoge nic position(or lateral).Conclusion We claims that brain CT-test should not replace EEG-test,because of both EEG-test and brain CT-test h ave superior or inferior,respectively,the use of combine EEG-test with brain CT- test are recommended,and the results of this paper provide advantageous referenc es for clinical practice.
