摘 要:目的 探讨大剂量氯氮平对心电图QT离散度(QTd)的影响。方法测定30例使用大剂量氯氮平(500 mg/日以上)治疗的精神病人的QTd及校正的QTd(QTcd),并以30例同期住院的神经内科病人作对照。结果 氯氮平组QTd、QTcd与对照组相比均明显增大,且具有显著差异,(QTd分别为(64.86±15.20)ms,(30.32±11.84)ms,P<0.01;QTcd分别为(72.73±15.82)ms,(32.43±10.56)ms,P<0.01。结论 大剂量氯氮平对心肌复极过程有影响,提示QTd、QTcd对预测猝死有一定的意义。Objectives: To examine the effects of high dose clozapine on the QTd of ECG. Method: The QTd and corrected QTd (QTcd) of ECG were measured in 30 psychotic patients treated with a high dose clozapine (>500 mg/d) and compared with those of 30 simultaneously hopitalized patients with neurological diseases. Results: The QTd and QTcd were increased significantly more greatly in clozapine group than in controlled group (QTd: (64. 86 ± 15. 20)ms verse (30. 32± 11. 84)ms, P<0.01;QTcd; (72. 73± 15. 82)ms verse (32. 43 ± 10. 56)ms, P<0. 01. Conclusion: Large dosage of clozapine affects the depolarization of cardiac muslces, suggesting QTd and QTcd measurements play some roles in predicting sudden death.
