子宫内膜癌并存心脑血管病老年患者的围手术期处理

摘　　要：目的　探讨子宫内膜癌并存心脑血管疾病老年患者的围手术期处理。方法　分析我院收治的 6 0例子宫内膜癌老年患者的临床资料及随访结果。术前并存心脑血管病为研究组 36例 ,无心脑血管病为对照组 2 4例。结果　6 0岁以上子宫内膜癌患者共 6 3例 ,3例因故未行手术 ,6 0例经以手术为主的综合治疗。研究组和对照组均无手术及术后近期死亡病例。平均住院天数分别为 2 9d和 19d (P<0 .0 5 ) ;手术及术后并发症分别为 14和 6例次 (P<0 .0 5 )。两组患者的平均手术时间 ,术中出血量及输血量 ,手术病理分期 ,肿瘤组织学类型 ,术后 3年及 5年生存率比较 ,差异无显著性意义。结论　子宫内膜癌伴心脑血管疾病的老年患者 ,在围手术期积极治疗并存病 。Objective To evaluate the efficacy of the perioperative management of concomitant cerebrovascular and/ar cardiovascular diseases in elderly patients with endometrial carcinoma.Methods The materials of 60 patients with endometrial carcinoma between 1980 and 2000 were retrospectively analyzed.The patients were divided into two groups according to the dividing point of concomitant cerebrovascular and/or cardiovascular diseases.Thirty six of them were identified to have concomitant cerebrovascular cerebr and/or cardiovascular diseases and the other 24 cases had no complicated diseases.Results Mean length of hospital stay of two groups was 29 days and 19 days,respectively(P< 0.05 ),and intra operative and postoperative complications occurred in 14 cases and 6cases,respectively,(P<0.05).Three and five year survival rates showed no significant difference.Conclusions The elderly patients with endometrial carcinoma and concomitant cerebrovascular and/or cardiovascular diseases can tolerate complicated gynecologic surgery in condition of controlling the perioperative concomitant diseases.
