缺血预处理对兔缺血再灌注脊髓血流量的影响

摘　　要：目的　研究缺血预处理 (IP)对缺血再灌注早期兔脊髓血流量的影响。方法　往置入腹主动脉的Swan Ganz导管气囊内注气建立兔脊髓缺血模型。将实验兔分为缺血组、预处理组和假手术组。应用激光多普勒血流量图像仪测量缺血期及再灌注 4h内的腰髓血流量。结果　在缺血 30min时 ,缺血组兔腰髓血流量 (2 3.0 % )和预处理组兔腰髓血流量 (36 .4 % )与各自基础值 (10 0 % )相比均有明显下降 (P <0 .0 1)。再灌注刚开始时 ,缺血组和预处理组兔腰髓血流量迅速回升 ,并超过基础值。再灌注 5 0min后 ,缺血组与预处理组兔相比 ,腰髓血流量 (85 .9% ,114 .3% )开始明显下降 (P <0 .0 5 )。再灌注 90min以后下降更为显著 (P <0 .0 1)。预处理组兔腰髓血流量在再灌注 4h内均高于基础值 ;假手术组兔腰髓血流量一直在基础值波动。Objective To study the effect of ischemic preconditioning(IP)on lumbar spinal cord blood flow during the early period of reperfusion after transient ischemia in rabbits.Methods The spinal cord ischemic model was established by inflating the balloon of a 4F Swan Ganz catheter positioned in the abdominal aorta of the rabbits.All rabbits were divided into sham operated group,ischemic group and IP group.Laser Doppler perfusion imager(LDI) was used to measure the lumbar spinal cord blood flow(SCBF) during the period of occlusion and the early 4 hours of reperfusion.Results Compared with their respective baseline values,the lumbar SCBF during the period of occlusion decreased markedly in the rabbits of ischemic and IP groups(P< 0.01 ). At the beginning of reperfusion, the lumbar SCBF of the rabbits in these two groups increased promptly and preponderated over the baseline values. After 50 minutes of reperfusion, the lumbar SCBF of the rabbits in ischemic group began to decrease obviously as compared with IP group(P<0.05) and it went a step further after 90 minutes of reperfusion(P<0.01).The lumbar SCBF of the rabbits in IP group exceeded the baseline value all through the 4 hours of reperfusion.Conclusions IP can improve SCBF during the early period of reperfusion after transient ischemia in rabbits and may protect spinal cord from ischemia reperfusion injury.
