摘  要:目的 探讨肺癌脱落细胞端粒酶活性检测的临床意义。方法 收集 63例肺癌患者和 3 1例非肺癌肺疾病患者的支气管肺泡灌洗液 ,同时行刷片和灌洗液细胞学检查 ;3 4例肺癌患者痰液 ,3 1例非肺癌肺疾病患者痰液 ;采用PCR TRAP银染法检测端粒酶活性。结果 肺癌和非肺癌肺疾病支气管肺泡灌洗液端粒酶活性阳性率分别为 76.2 % (4 8/63 )和 6.5 % (2 /3 1) ,P <0 .0 1;肺癌支气管肺泡灌洗液端粒酶活性阳性率高于刷片细胞学阳性率 (5 8.7% ,3 7/63 ) ,P >0 .0 5 ;高于灌洗液细胞学阳性率(14 .7% ,9/63 ) ,P <0 .0 1。肺癌痰液和非肺癌肺疾病痰液端粒酶活性阳性率分别为 2 9.4% (10 /3 4) ,3 .2 % (1/3 1) ,P <0 .0 1。结论肺癌端粒酶活性检测有较高的特异性和敏感性 ,可应用于肺癌的临床诊断。支气管肺泡灌洗液和痰液端粒酶检测能提高肺癌检出率。Objective To access the clinic significance of telomerase activity in exfoliated cells of bronchoaleolar lavage fluid(BALF) and sputum from patients with lung cancer.Methods 63 BALF of lung cancer and 31 BALF of non lung cancer lung diseases were collected.At the same time the brush cytology and BALF cytology examination were carried out.34 sputum of lung cancer and 31 sputum of non lung cancer lung disease were collected.The telomerase activity was measured with the modified telomeric repeats amplification protocol(TRAP).Results Telomerase activity was present in 48 out of 63 BALF of lung cancer,2 out of 31 BALF of non lung cancer lung diseases( P <0.01);10 out of 34 sputum of lung cancer,1 out of 31 sputum of non lung cancer lung disease( P <0.01).The positive rates of brush cytology and BALF cytology were 58.7%(37/63) and 14.3%(9/63) respectively,lower than that of telomerase activity detection of BALF( P >0.05, P <0.01).Conclusion Telomerase activity is present in exfoliated cells of BALF and sputum.Telomerase activity detection have high sensitivity and high specificity for diagnosis of lung cancer.The telomerase detection of BALF and sputum is useful for diagnosis of lung cancer.
