细胞间粘附分子-1在原发性肝癌的检测及意义

摘　　要：研究ICAM - 1在HCC的意义。以免疫组化方法结合全自动图像分析仪检测ICAM - 1在HCC组织的表达 ;以ELISA法测定HCC患者血清中sICAM - 1水平。 1 肝癌组织中ICAM - 1含量明显高于正常肝组织 ,转移组肝癌ICAM - 1含量明显高于无转移组。 2 肝癌患者血清sICAM - 1水平显著升高 ,并与肝功能损害程度、肿瘤大小呈正相关 ,Ⅲ、Ⅳ期患者sICAM - 1水平高于Ⅱ期患者 ,癌肿数目≥ 3个的患者sICAM - 1水平高于癌肿数目 <3个的患者 ,有门脉癌栓及肝内外转移的患者sICAM - 1水平高于无门脉癌栓及肝内外转移的患者 ,有两例HCC患者行TAE治疗后 ,病情缓解 ,sICAM - 1水平下降。HCC组织高度表达ICAM - 1,其表达量与肿瘤转移状态有关。肝癌患者血清中ICAM - 1的检测可作为肝癌发展程度、转移状态及预后判断的一个推测指标。To study the expression and significance of intercellular adhesion molecule-1 (ICAM-1) in human hepatocellular carcinoma(HCC). Immunohistochemical location of ICAM-1 was performed on liver tissue sections of patients with HCC. Serum levels of soluble intercellular adhesion molecule-1(sICAM-1)were measured in patients with HCC. 1.Tissue ICAM-1 was higher in liver cancer than in normal tissue. Tissue ICAM-1 was higher in liver cancer with metastasis than in liver cancer without metastasis. 2. Serum levels of sICAM-1 in patients with HCC were significantly higher than in those of LC patients and normal controls. In patients with HCC, levels of sICAM-1 directly correlated with tumor volume, liver damage degree. Furthermore, serume sICAM-1 levels were higher in patients with multiple HCC(tumor number≥3)than in those with tumor number<3, and the levels of sICAM-1 were higher in HCC patients with stage Ⅲ or Ⅳ than in those with stage Ⅱ. sICAM-1 levels were significantly elevated in patients with tumor embolus in portal vein or with metastasis. The sICAM-1 levels decreased with the improvent of patients' condition during the course of treatment in 2 HCC patients. Enhancement of ICAM-1 expression was found on hepatoma tissues. And the levels of tissue ICAM-1 directly correlated with tumor metastasis. There were significantly high level of sICAM-1 in serum of HCC patients. sICAM-1 could be a marker of speculate the disease progress, metastasis and curative effect of HCC patients.
