中枢神经细胞瘤病理特征研究及文献复习

摘　　要：目的　本文报道 7例脑中枢神经细胞瘤 ,复习文献并探讨其组织学、免疫组化和超微结构特点。方法　复习 1993- 2 0 0 1年间 17例脑室肿瘤 ,进行组织学观察 ,用免疫组化检测几种重要的肿瘤标记物的表达 ,并对部分病例进行电镜观察。通过对PCNA染色和DNA含量图像分析 ,研究本瘤的增殖活性和DNA倍体。结果　 17例脑室肿瘤中 ,7例确诊为中枢神经细胞瘤。组织学检查该肿瘤具有一定的形态学特点如由大小一致的圆形细胞组成 ,核圆形 ,点彩状 ,细胞核周有明显的空晕 ,可见神经纤维小岛等。免疫组化结果显示全部病例均为Syn阳性。电镜观察可见较多的神经内分泌颗粒及含微管结构的突触。PCNA染色结果显示肿瘤细胞增殖指数 <5 %。DNA含量分析显示 ,7例中枢神经细胞瘤平均 78%为 2倍体细胞 ,提示为良性肿瘤。结论　中枢神经细胞瘤具有一定的组织学特征以及免疫组化和电镜特点 。Objective To report 7 cases of central neurocytoma with review of the literature,and to discuss its histological,immunohistochemical,and electron microscopic features. Methods Total 17 cases of ventricle tumors between the year of 1993 and 2001 were reviewed,and their histological features and the expression of several tumor markers were investigated using immunohistochemistry,and electron microscopic observation in several cases.Proliferative index and DNA content in the tumor cells were measured through PCNA immunohistochemiscal staining and DNA content image analysis. Results Among 17 cases of ventricle tumors,7 cases were diagnosed as central neurocytoma. The tumors had histological features such as composition of round cells of the similar size, round and pepper-like nucleus, clear zone surrounding the nucleus and presence of neurofibril islands. Synapsin was positive in all the cases. Ultrastructural features included many secretory vehicles and synapse containing microtubules .PCNA immunohistochemistry showed proliferative index of the tumors were all below 5%. DNA content analysis showed an average of 78% diploid cells in 7 cases of the tumor,indicating they were benign. Conclusion It is very important for making a correct diagnosis to know the histological, immunohistochemical and electron microscopic features of central neurocytoma.
