血清急性时相蛋白在胃癌中的表达及临床应用价值

摘　　要：目的 :探讨血清急性时相蛋白在胃癌中发生、发展中的作用及临床意义。方法 :手术前用Array特定蛋白分析系统 ,全自动速率散射比浊法检测 12 0例胃癌患者的C 反应蛋白 (CRP)、α1 抗糜蛋白 (AT)和α 酸性糖蛋白 (α AG)水平。结果 :CRP、α1 AT和α AG的阳性表达明显高于良性胃病组 (P <0 0 1)。与肿瘤患者的年龄、性别、部位及细胞的分化程度无明显相关 (P >0 0 5 ) ;侵犯胃壁浆膜及肌层组显著高于粘膜及粘膜下层组 (P <0 0 1) ;有淋巴结转移组明显高于无淋巴结转移组 (P <0 0 1) ;Ⅲ、Ⅳ期显著高于Ⅰ、Ⅱ期 (P <0 0 1)。随访表明其阳性组复发率 ( 82 3%、87 5 %、88 2 % )与阴性组 ( 30 2 %、2 9 6 %、2 7 9% )比较 ,差异有显著性 (P <0 0 1)。结论 :CRP、α1 AT和α AG是胃癌发展过程中的一种有价值的病理指标 ,也可作为判断胃癌预后的指标之一。Objective: To investigate the clinical implication and effects of expression of CRP?α 1-AT and α-AG in thesignificant and development of gastric cancer.Methods: CRP?α 1-AT and α-AG expression were detected in 120 specimens of gastric cancer before curative gastric surgery.Results: The positive rate of C-reactive protein (CRP)?α 1-antitrypsin(α 1-AT) and α-acid glycoprotein (α-AG)expression in gastric cancer group were significantly higher than that in control group (P<0.01).The expression of CRP?α 1-AT and α-AG were not correlated with age?sex?and cells degree of differentiation(P>0.05).Protein levels of CRP?α 1-AT and α-AG in invasion gastric chorion was higher than that of mucous grouop(P<0.01);protein levels of CRP?α 1-AT and α-AG in lymph nodes metastases group overexpressed (P<0.01);protein levels of CRP?α 1-AT and α-AG in the stage Ⅲ and Ⅳ tumors was higher than that in stage Ⅰ?Ⅱ tumors(P<0.01).There were significant differences in recurrence rates between C-reactive protein response(82.3%),α 1-AT response (87.5%),α-AG response(88.2%) and those without such a response accordingly (P<0.01).Conclusion: The CRP?α 1-AT and α-AG might play a great role in the occurrence and development of gastric cancer,and could used as an indicator for evaluating the prognosis of gastric cancer.
