肾透明细胞癌的CT表现与癌细胞核DNA含量的关系

摘　　要：目的　探讨肾透明细胞癌的CT表现与癌细胞核DNA含量的相关性。方法　搜集 5 8例术前已行CT检查与经手术病理证实为肾透明细胞癌者 ,比较癌细胞核DNA含量与CT表现的关系。结果　 (1)肿瘤直径 >5 0cm者其癌细胞核DNA含量明显高于≤ 5 0cm者 (t=5 86 0 ,P <0 0 1)。 (2 )CT表现有坏死、液化、囊变 ,边界不清的肿瘤 ,其DNA含量显著高于无坏死、液化、囊变 ,边界清晰的肿瘤 (t=5 95 2 ,P <0 0 0 1)。 (3)CT发现有淋巴结转移 ,肾静脉或下腔静脉受侵 ,邻近器官侵犯或远处转移者 ,其DNA含量显著高于无淋巴结转移、肾静脉或下腔静脉未受侵犯、无邻近器官侵犯或远处转移者 (t=4 90 9、4 4 6 2 ,P <0 0 0 1)。 (4)肾周脂肪是否受累 ,肿瘤强化明显或不明显 ,其DNA含量无显著性差异 (t=0 791、0 4 87,P >0 0 5 )。结论　肾透明细胞癌直径 >5 0cm ,肿瘤内出现坏死、液化、囊变 ,边界不清 ,有淋巴结转移 ,肾静脉或下腔静脉受累 ,邻近器官侵犯或远处转移者其DNA含量明显增高 ,肿瘤生物学行为恶性度更大。Objective To study the correlation of CT appearance with nuclear DNA content in renal clear cell carcinomas Methods Fifty eight cases of renal clear cell carcinomas proved by surgery and pathology were examined with abdominal CT scan before operation DNA content was determined by imaging analyzer, and DNA contents were calculated Study on the correlation between CT appearance and nuclear DNA content was performed Results (1) DNA contents of tumors with diameters >5 0 cm were significantly higher than those of tumors with diameters ≤5 0 cm ( t =5 860, P <0 01) (2) DNA contents in tumors with intratumoral necrosis, liquefaction, cystic degeneration or poorly defined margins on CT appearance were significantly higher than those of tumors without intratumoral necrosis, liquefaction or cystic degeneration, and with clearly defined margins ( t =5 952, P <0 001) (3) DNA contents in tumors with lymph nodes metastases, invasion of renal vein or inferior vena cava, invasion of adjacent organs or distant metastases were significantly higher than those of tumors without lymph nodes metastases, without invasion of renal vein or inferior vena cava, without invasion of adjacent organs or distant metastases ( t =4 909,4 462, P <0 001) (4) There was no significant difference in DNA contents between tumors with perirenal fat invasion and tumors without perirenal fat invasion So is the case in the enhancement degree of tumors ( t =0 791,0 487, P >0 05) Conclusion Renal clear cell carcinomas with diameters >5 0 cm, intratumoral necrosis, liquefaction, cystic degeneration, lymph nodes metastases, invasion of renal vein or inferior vena cava, invasion of adjacent organs or distant metastases had higher DNA content Those tumors had higher malignant biological behavior
