角膜内皮细胞化学性损伤及功能失代偿的实验研究

摘　　要：目的 :研究引起角膜内皮细胞化学性损伤的因素 ,探讨不同化学物质对角膜内皮细胞的影响及预防措施。方法 :将不同的化学物质用前房内注射、前房内灌注和玻璃体内注射等 3种方法 ,制成兔角膜内皮细胞化学性损伤动物模型 ,用药后 1 5d ,对兔角膜行裂隙灯检查 ,观察角膜内皮细胞形态和数量及超微结构 ,分析不同化学物质对角膜内皮细胞的影响。结果 :高浓度庆大霉素 (2 5mg/ 0 .2ml)及未经冲洗含有戊二醛的注吸头前房内注射的生理盐水 ,对角膜内皮细胞造成重度损伤 ;Healon,麦斯特 ,生理盐水及Bssplus前房内注射 ,不会损伤角膜内皮细胞 ;硅油 ,重水 ,过氧化烷玻璃体内注射对角膜内皮细胞无影响。结论 :1 角膜内皮细胞的化学性损伤 ,与眼内注入化学性物质的浓度和时间密切相关 ;2 Bssplus、Healon和生理盐水的短时间灌注对角膜内皮细胞没有损伤 ;3 统一用药和器械消毒的标准 。Objective:To study the factors that induce the chemical destruction of corneal endothelium,to discuss the influence of different chemical substances and prevention the syndrome. Method:Transmit different substances into rabbits' eyes using the following three ways:intrachambel injection,intrachambel irrigation and intravetrious injection.After 15 days,the physical checkup by slit lamp were undergone to every eyein,and made the exam on morphologic and quantity of corneal endothelium and substructure exam at the same time.Analysis the influence of different substances to the corneal endothelium.Results:The corneal endothelium should be destructed by the following substances:gentamycin in high consistence (25mg/0.2ml),the normal saline that injected by the pinhead without washing.Healon,Mester,normal Saline and Bssplus did not induce destruction to corneal endothelium.Silicon,heavy water and C 3F 8 were safe to the corneal endothelium.Conclusion:(1)The chemical destruction of the corneal endothelium is related to the consistence and time of chemical substance intimately.(2)Irrigation in short time of the Bssplus,Healon and normal saline are safe to the corneal endothelium.(3)It's very important to prevent the chemical destruction to corneal endothelium,and establish an united standard of administration and instrument sterilization.
