摘  要:背景与目的:食管粘膜碘染色可提高早期食管癌和癌前病变的内镜分辨率,但是,由于染色机理不明而影响此技术的推广。本文拟研究食管癌组织中糖原及含糖物质的变化和食管上皮组织碘染色机理。方法:在胃镜下行食管上皮碘染色,根据上皮细胞着色情况诊断早期食管癌及癌前病变。采用β-甘油磷酸缓冲液提取食管上皮组织中糖原或含糖物质。采用糖原染色法(periodicacid-shiffstaining,PAS染色)检测组织切片糖原的存在及含量。采用组织匀浆法提取组织中蛋白质成分。结果:经胃镜行食管上皮碘染色后,正常食管粘膜呈深棕色着色,而早期食管癌和癌前病变则呈界限分明的浅黄色区域。正常食管上皮组织切片经PAS染色,其中的糖原被染成紫红色颗粒,但经淀粉酶消化后的组织切片则不显色;而食管癌及癌前病变组织切片无论经淀粉酶消化与否均不着色。食管粘膜组织中糖原提取结果显示,肿瘤组织比正常组织糖原含量显著降低(P<0.01),但组织提取液中的蛋白质含量在正常食管粘膜组织与肿瘤组织相比无显著差别(P>0.05)。结论:食管癌及癌前病变组织中糖原或含糖物质含量明显降低,经碘染色后成为浅染区,这可为肿瘤诊断提供依据。Background &Objective:It was known that mucosal iodine staining could be used to improve the endoscopic visualization of esophageal premalignant lesion and cancer.However,the method was not used wid ely because of lacking clear mechanism.This study was designed to investigate the mech anism of esophageal epithelium staining with iodine and changes of glycogen or sac chariferous materials.Methods:Esophageal epithelium was stained w ith iodine in endoscopic examination.According to color of tissue,the esophageal ca rcinoma and premalignant lesion were diagnosed.Glycogen from the esophageal mucous epithelium was extracted withβ-glycerophosphate buffer.The cont ent of glycogen in the tissue speciman was detected by p eriodic acid-shiff staining(PAS).Protein in the esophageal mucous ep ithelium was extracted by tissue homogenate meth od.Results:After iodine staining,the normal ep ithelium became brown and easier to distinguish normal epithelium from precancerous or premalignant epith elium according to the change of colo r.Furthermore,normal epithelium slice stained by P AS technique had red reaction but it h ad no reaction after pretreatment by amylase.However,precancerous and premalig nant epithelium tissue slices could not be stained whenever they we re pretreated by amylase.The content of glycogen extracted fr om the esophageal epithelium was lower significantly than those o f normal tissue(P<0.01).But it was no deviation that protein c ontents of normal esophageal epithelium were compared with carcinoma tissue(P>0.05).Conclusion:These results demonstrated that gly cogen and sacchariferous materials in esophageal carcinoma and premalignant lesion tissue decreased significantly and result in light yellow area by iodine staining.It ma y be helpful for diagnosis of esophageal carcinoma.
