虹膜型人工晶状体植入术并发症的原因分析

摘　　要：目的 :探讨虹膜型人工晶状体植入术并发症的原因。方法 :综合分析 6 7例 6 7只眼虹膜型人工晶状体植入手术的术中、术后并发症 ,追踪观察并发症的远期结果对视力的影响。结果 :术后并发症最常见为眼压升高 38 8% (2 6 / 6 7) ,其次为角膜水肿 7 46 % (5 / 6 7)、前房出血 7 46 % (5 / 6 7)、脉络膜脱离 5 97% (4 / 6 7)、视网膜脱离 4 47% (3/ 6 7)、玻璃体出血 1 49%(1/ 6 7)。结论 :虹膜型人工晶状体植入术并发症的原因复杂 ,尤以眼压升高为主 ,远期对视力有一定影响 。Objective:To analyse the complications after black-diaphragm intraocular lens implantation.Methods:Black-diaphragm intraocular lens implantation was done in 67 patients (67 eyes) with extensive loss of iris tissue.Preoperative and postoperative IOP,corrected vision and other complications were observed respectively.Results:Improvement of visual acuity was observed in most patients.In 26 of 67(38.8%)cases,the postoperative IOP were 25～50mmHg within 8 days.Of 8 patients who had a history of secondary glaucoma preoperatively,2 patients had pressure uncontrolled medically after IOL implantation and underwent anti-glaucoma operation again.Of the 59 patients who had no history of preoperative glaucoma,7 patients had glaucoma which was controlled well medically during 1～15 months follow-up.2 patients developed retinal detachment and had IOP removed.Conclusion:In most of the patients with traumatic aniridia,implantation of black-diaphragm aniridia intraocular lens may improve the visual acuity.Increased IOP was one of the greatest postoperative complications.
