摘　　要：目的　探讨微创伤冠状动脉搭桥术在老年患者的应用体会。方法　胸部正中切口 ,常温、非体外循环下行冠状动脉搭桥术 2 6例(平均 65.6岁 )。 C TS固定器降低靶血管的活动度 ,环绕缝合法阻断切开的冠状动脉血流。结果　术后无严重并发症和死亡 ,呼吸机辅助时间、住监护病房时间和平均输血量明显减少。随访 1～ 30个月 ,无心绞痛复发 ,生活质量明显提高。结论　非体外循环下冠状动脉搭桥术治疗老年冠心病创伤小、安全。Objectives To discuss the experience in minimally invasive coronary artery bypass grafting in elderly patients.Methods From January 1999 to June 2000, 26 patients, aged 60 years and over, underwent off pump coronary artery bypass grafting through median sternotomy. Retracting sutures of pericardium, wet guaze pads and CTS heart stabilizer were placed for the purposes of exposure and fixation of target vessels. Target vessels were surrounded by two looping sutures proximal and distal to the chosen sites for anastomosis.Results There were no hospital death, no perioperative myocardial infarction and other major complications. The patients had a shorter duration of postoperative intubation, a shorter intensive care unit stay, and lower blood transfusion requirements. All of the 26 patients showed improvement in quality of life and had no recurrent angina during a follow up of 1 to 30 months. Conclusions Off pump coronary artery bypass grafting for elderly patients appears to be minimally invasive? safe and effective.
