经食管超声心动图声学密度定量技术评价梗死心肌的临床研究

摘　　要：目的　探讨多平面经食管超声心动图 (TEE)声学密度定量技术 (AD)定量评价梗死心肌的可行性。方法　对 2 0例心肌梗死患者和 2 0例正常人 ,用TEE和经胸超声心动图 (TTE)分别测量其背向散射积分 (IBS)及其心动周期变异幅度 (CVIB) ,并与放射性核素心肌显像 (ECT)进行相关性分析。结果TEE测量梗死心肌的IBS明显高于正常心肌和TTE的测值 (P <0 .0 0 1) ,而CVIB则明显减低 (P <0 .0 1) ;TEE的AD值同99mTc MIBI心肌显像检查的心肌活性比率高度相关 (r =0 .93 ) ,高于TTE(r =0 .81)。bjective To evaluate the value of acoustic densitometry(AD) technique in detecting infarcted myocardium by transesophageal echocardiography(TEE).Methods Twenty patients with anterior interventricular septum myocardial infarct and 20 normals were performed standard TEE examination and AD analysis in the short axis view of left ventricle at the level of papillary muscles . The integrated backscatter(IBS) and the magnitude of cyclic variation of IBS(CVIB) were obtained from the anterior inter ventricular septum and the left ventricular posterior wall, and compared with that of transthoracic echocardiography(TTE). Results The value of IBS of infarcted myocardium measured by TEE was higher than that of normal and TTE(P< 0.00l ), however, the value of CVIB was lower(P< 0.0l ). In addition, the value of IBS and CVIB measured by TEE correlated well with the activity ratio on Tc 99m sestamibi single photon emission computer tomography (r= 0.93 ), and the correlation was better than TTE(r= 0.81 ).Conclusions AD technique by TEE is a new feasible method for quantitative assessment of infarcted myocardium.
