VIP-CT瓣在美学区即刻种植即刻修复中的临床应用

摘　　要：目的探讨采用血管化骨膜-结缔组织夹层(VIP-CT)瓣在美学区即刻种植即刻修复中的临床效果。方法选择合适的上颌前牙区单牙微创拔牙后即刻种植即刻修复患者共30例,均采用VIP-CT瓣关闭拔牙创口,制作临时冠诱导牙龈成形,且术后3个月左右行最终修复。修复完成后当天、6个月及12个月,以粉红美学指数(PES)对种植体周围软组织进行美学评价;同时以视觉模拟评分法(VAS)对患者进行主观满意度调查。结果在追踪评估期间内,2例患者未按时复诊退出评估,剩余28例患者各时期的PES评分分别为(12.23±1.56)、(11.47±1.42)和(11.35±1.21)分,术后创面均完全封闭,唇侧丰满度较好,种植体周围软组织形态恢复正常,患者满意度较高。结论在美学区即刻种植即刻修复中采用VIP-CT瓣可以获得良好的美学修复效果。Objective To study the clinical effect of vascularized inter-positional periosteal-connective tissue( VIP-CT flap) on the aesthetic area immediately implanted and immediately prosthesis. Methods A total of 30 patients with minimally invasive tooth extraction immediately after implant of single tooth and immediate prosthesis in maxillary anterior teeth area were chosen,VIP-CT flap was adopted in closing tooth extraction wound,temporary crowns were made to induce gum forming,and the final repair was performed postoperative 3 months.After completion of repair on the very day,6 months and 12 months,pink aesthetic score( PES) was used to evaluate aesthetically the soft tissue around the implant. At the same time,the method of visual analogue scale( VAS) was carried out to investigate the subjective satisfaction of the patients. Results During the period of tracking assessment,two patients who did not visit exited the evaluation,PES grading of 28 patients in each period was respectively( 12. 23 ± 1. 56),( 11. 47 ± 1. 42),( 11. 35 ± 1. 21). The postoperative wounds were completely closed,labial side was in better fullness,the surrounding soft tissue forms of implant returned to normal,and the patients were satisfied. Conclusion The application of VIP-CT flap can obtain good aesthetic repair effect in immediate implant and immediate prosthesis for patients.
