摘  要:目的评价端粒酶活性作为非小细胞肺癌标志物及对其预后的判断。方法 采用纤维支气管镜下毛刷支气管粘膜脱落细胞 ,应用端粒酶重复放大程序 (Telomeraserepeatamplificationprotocol,TRAP)检测60例非小细胞肺癌和20例肺部炎症患者支气管粘膜脱落细胞端粒酶活性。结果 非小细胞肺癌组患者患侧支气管粘膜脱落细胞端粒酶定性检测阳性53例 (88.3 % ) ,对侧端粒酶检测阳性5例 (20 % ) ;肺炎症组端粒酶活性检测阳性2例(10.0% ) ,分别与肺癌患侧及对侧比较差异显著 (χ2=33.2,P<0.05)。定量法检测非小细胞肺癌的端粒酶活性平均A值0.1098 ,明显高于肺炎症组平均A值0.018(U=4.95 ,P<0.05)。Ⅰ~Ⅲa期肺癌患者端粒酶活性水平为0.132 ,低于Ⅲb~Ⅳ期活性水平0.173(U=1.899,P<0.05)。结论 端粒酶活性检测有可能作为非小细胞肺癌的标志物。端粒酶活性水平随非小细胞肺癌分期增加而增加 ,可为患者预后提供判断依据。Objective To evaluate the clinical significance of telomerase activity in fibrobronchoscopic brushing cells in prognosis of non_small cell lung cancer(NSCLC).Methods The authors studied the exfoliated cells from fibrobronchoscopic brushing using a ploymerase chain reaction based on a telomerase repeat amplification protocal assay.Samples were taken from 60 cases of NSCLC and 20 cases of pulmonary infection.Results Telomerase activity was detected in 53 of 60(88.3%) NSCLC specimens from the lesion side and in 5 of 25(20.0%) from the contralateral side but only in 2 of 20 specimens of pulmonary infection(χ2=33.21,P<0.05).The telomerase activity levels in NSCLC (meduim 0.1098) were significantly higher than those in pulmonary infection (medium 0.018,U=4.95,P<0.05).Telomerase activity levels in tumor stage Ⅲb~Ⅳ(medium 0.173) were higher than that in stage Ⅰ-Ⅲa(medium 0.132,U=1.899,P<0.05).Conclusions Telomerase activity is one of the important markers in patients with NSCLC.Telomerase activity increased with the advance of tumor stage and can be used as a prognostic indicator of advanced NSCLC.
