多点微创硬管抽吸引流治疗重症高血压脑出血

摘　　要：目的　探讨用多点微创抽吸法解除重症高血压脑出血急性期颅压高的方法。方法　 39例重症病例依CT所见分型 ,用微创穿颅置硬导管抽吸引流方法治疗 ,分别在血肿中三点置管 2例 ,血肿两点置管 15例 ,双侧脑室额角置管 8例 ,血肿一点加一侧脑室置管 11例 ,血肿两点置管加对侧脑室置管 2例 ,双侧室体部置管 1例。结果　 39例全部存活。ADL分级 :Ⅰ级 6例 ;Ⅱ级 10例 ;Ⅲ级 12例 ;Ⅳ级 8例 ;Ⅴ级 3例。结论　多点微创硬管抽吸引流是缓解重症高血压脑出血急性期颅压高。Objective To investigate the treatment of higher intraeranial(encephalic) pressure of acute cerebral hemorrhage by use of multi-spot hard tube minimal incision perforation.Methods Breaking down the different mechoces based on past 39 clinical cases of severe hypertensive intracerebral hemorrhage which was classified by the appearance on CT.Drawing of intracranial hematoma and drainage of ventricular are principled technics:2 cases placing hard drainage tubes at three spots of hematoma;15 case with two spots of hematoma;8 cases with both sides intraventricular drainage;11 cases with one spot of hematoma plus one side intraventricular drainage;2 cases with two spots of hematoma and opposite side intraventricular drainage;1 case with both sides intraventricular drainage at pars centralis.Result ADL Grade:1 Grade 6 cases;2 Grade 10 cases;3 Grade 12 cases;4 Grade 8 cases;5 Grade 3 cases.Conclusion Drawing and drainage with multi-spot minimal incision perforation is a simple and effective method in treating severe hypertensive intracerebral hemorrhage.
