CD44v6在子宫内膜腺癌中的表达及其意义的研究

摘　　要：目的　探讨CD4 4v6在子宫内膜腺癌发生、发展中的作用。方法　本实验运用免疫组化技术SP法检测CD4 4v6在子宫内膜腺癌 5 5例、正常增生期子宫内膜、单纯性增生和不典型增生各 10例中的表达 ,分析其与子宫内膜腺癌临床病理参数及患者生存率的关系。结果　 (1)在正常增生期子宫内膜、单纯性增生、不典型增生、子宫内膜腺癌中CD4 4v6阳性表达率分别为 10 %、4 0 %、6 0 %、78.18% ,统计学上有显著性差异 (P <0 .0 0 1) ,5 5例子宫内膜腺癌组织中 ,CD4 4v6表达强度与手术分期及淋巴结有无转移呈负相关 (P <0 .0 5 ,P <0 .0 1) ,与肌层浸润、分化程度无关。 (2 )单因素生存分析CD4 4v6与子宫内膜腺癌的预后有关 (P <0 .0 0 1) ,多因素Cox模型分析筛选出患者的年龄、手术分期和CD4 4v6是独立的预后影响因素 ,CD4 4v6阴性表达患者较阳性表达者生存时间短。结论　CD4 4v6在子宫内膜腺癌的发生、发展过程中起一定的作用 ,子宫内膜腺癌CD4objective The aim is to study the effects of the gene expression of CD44v6 on oncogenesis and progression of endometrial carcinoma.Methods The expression levels of CD44v6 were studied by S-P immunohistochemistry method in 55 cases of endometrial adenocarcinoma and 10 cases of normal proliferatic,simple hyperplastic and atypical endometrium respectively.Their relationship relating with clinicopathological features and surrival was also investigated.Results (1)The expression of CD44v6 increased progressively from normal proliferatic,simple hyperplastic and atypical hyperplastic endometrium to adenocarcinoma,whose ratios were 10%,40%,60% and 78.18% respectively,showing high significant difference( P <0.001),The CD44v6 expression in 55 cases of carcinoma was inversely associated with surgical stages and metastasis of lymph nodes ( P<0.05,P <0.01),while no significant difference was detected in different myometrial invasion and histologic grade.(2)the analysis of single factor revealed that the expression of CD44v6 was significantly related with the prognosis of patients ( P <0.001).Analyzed by Cox proportion hazard model,it showed that the prognosis was independently affected by age,surgical surgical stage and CD44v6,and that the prognosis of patients with negative expression of CD44v6,and that the prognosis of patients with negative expression of CD44v6 had a shorter average survival time than that had positive expression.Conclusion The experiment suggests that CD44v6 play roles in oncogenesis and progression of endometrial adenocarcinoma.The negative expression of CD44v6 indicate poor prognosis of endometrial adenocarcinoma
