单剂欧必亭加地塞米松预防大剂量顺铂所致的恶心呕吐

摘　　要：目的　研究单剂欧必亭加地塞米松预防大剂量顺铂化疗所致恶心呕吐的疗效。方法　将 93例接受大剂量顺铂化疗的患者随机分为欧必亭组和枢复宁组 ,分别于第 1天用顺铂前 30min静滴地塞米松 1 0mg ,静注欧必亭 5mg或枢复宁 8mg ,观察两组急性和迟发性恶心呕吐的控制率。 结果　欧必亭组预防急性恶心、呕吐的有效率分别为 97.9%和 95 .8% ,CR率均为 91 .6 % ;预防迟发性恶心、呕吐的有效率均在 80 %以上 ,但CR率偏低 ,有超过一半的患者其迟发性恶心未完全控制。与枢复宁组比较 ,除预防急性恶心的CR率欧必亭组优于枢复宁组外 ,其余指标两组均无显著性差异。结论　单剂欧必亭加地塞米松能有效预防大剂量顺铂化疗所致的急性恶心、呕吐 ,但迟发性恶心、呕吐的完全控制率偏低。Objective To evaluate the roles of single dose Tropisetron plus dexamethasone in the prevention of high dose cisplatin induced emesis.Methods 93 patients who received high dose cisplatin chemotherapy(60～80mg/m 2) were randomized into two groups:group A,Tropisetron 5mg i.v.plus dexamethasone 10mg i.v. by drip 30 minutes before cisplatin administration on day 1;group B,as in group A but with Zofran substituted for Tropisetron.The episodes of acute and delayed emesis of two groups were recorded.Results In group A,the overall response(OR) rate of acute nausea and vomiting was 97.9% and 95.8% respectively,the complete response(CR) rate being 91.6%.The OR rate of delayed emesis was more than 80% in group A,but the CR rate of delayed nausea was less than 50% in the same group.The CR rate of acute nausea was higher in group A than that in group B(P<0.05).The OR rate of acute and delayed emesis was comparable in these two groups.Conclusion Single dose Tropisetron plus dexamethasone can effectively control the acute emesis induced by high dose cisplatin.The CR rate of delayed emesis was less pronounced in this regimen.
