不同剂量甘露醇对高血压脑出血患者颅内压及肾功能的影响

摘　　要：目的　探讨甘露醇治疗高血压脑出血后脑水肿的合适剂量。方法　选择在我院住院治疗的高血压脑出血患者 58例 ,将其随机分为 3组 :全量组 (A组 ) ,半量倍次组 (B组 )和半量组 (C组 )。比较 3组降颅压效果、对神经功能及BUN、Cr的影响。结果　不同剂量甘露醇降颅压效果及对神经功能的影响在 3组间的差别均无显著性意义(P >0 0 5) ;用药后第 7天 3组BUN、Cr值间的差别均有显著性意义 (P <0 0 5)。结论　半量甘露醇与全量和半量倍次甘露醇的脱水效果、对神经功能恢复均相似 ,但对肾功能损伤较轻 ,故半量甘露醇是治疗高血压脑出血后脑水肿的合适剂量。Objective To study the suitable dosage in treating brain edema of the patients with hypertension intracerebral Hemorrhage.Methods 58 patients with hypertension intracerebral hemorrhage were randomly divided into three groups,full dosage group (A),half dosage and double time group(B) and half dosage group(C).Compared The effect of decreasing the intracranial pressure,the changes of nerves function and the content of BUN,Cr were compared among the three groups.Results There were no significant differences in the effect of decreasing the intracranial pressure and the influence on nerves function among the three groups(P>0 05).There were significant difference in the content of BUN and Cr among the three groups at 7th day(P<0 05).Conclusion Half dosage of mannitan had the same effect of decreasing the intracranial pressure and changes in nerves function as the full dosage or half dosage and double time,but had more slight injury to renal function.Accordingly,half dosage of mannitan was the suitable dosage in treating brain edema of the patients with hypertension intracerebral Hemorrhage.
