左旋卡尼汀对老年糖尿病合并高血压病患者糖脂代谢的影响

摘　　要：目的　探讨老年糖尿病患者血浆游离肉碱浓度 (PFC)的改变及国产左旋卡尼汀 (L CN)治疗对糖脂代谢的影响。方法　用国产L CN针剂治疗 2 0例老年 2型糖尿病伴高血压病患者 ,并以 2 0例老年高血压病患者为对照 ,测定治疗前后PFC、血糖及血脂的变化。结果　糖尿病伴高血压病患者治疗前PFC(5 0 .5 9± 13.4 1) μmol L低于高血压病患者 [(6 3.32± 15 .2 3) μmol L ,P <0 .0 1],经L CN治疗后升高至 (10 5 .74± 5 5 .83) μmol L ,与治疗前相比差异有显著性意义 (P <0 .0 0 1)。血糖也明显下降 (P <0 .0 0 5 ) ,血脂改变不明显 (P >0 .0 5 )。结论　老年 2型糖尿病伴高血压病患者PFC下降 ,体内可能有肉碱缺乏 ,外源性L CN的补充可明显提高PFC ,降低血糖 。Objective To investigate the change of plasma free carnitine level (PFC) in senile patients with diabetes mellitus and the influence of L carnitine on glucose and lipid metabolism. Methods PFC,blood glucose and lipid levels were measured in 20 senile type II diabetic patients with hypertension (group 1) and 20 patients with essential hypertension (group 2) before and after they received L carnitine 3.0 g intravenous infusion per day for 10 days. Results It was observed that PFC level was lower in group1 [(50.59±13.41) μmol/L] than in group 2 [(63.32±15.23) μmol/L,[WTBX]P<0.01)] before treatment.But the PFC level was elevated significantly before treatment.But the PFC level was elevated significantly [(105.74±55.83) μmol/L,[WTBX]P<0.001)] after treatment, accompanied by decrease in blood glucose level ( after treatment, accompanied by decrease in blood glucose level (P<0.005).But the changes of blood lipid were not significant.Conclusions It has been thought that carnitine deficiency maybe exist in type II diabetic patients,and it is useful to supply L carnitine for the patients with diabetes mellitus.
