摘 要:目的 研究一种新的抑癌基因PTEN在乳腺癌中的表达及意义。方法 应用免疫组织化学SP法 ,检测了 2 8例乳腺癌及2 1例癌旁乳腺组织PTEN蛋白的表达 ;EnVision法检测癌组织中雌、孕激素受体的表达。结果 2 1例癌旁乳腺组织中均有PTEN蛋白的表达 ,免疫组化染色较强 ,阳性物质位于乳腺小叶腺泡上皮细胞及导管上皮细胞的胞质和胞核。在乳腺癌组织中PTEN蛋白表达于癌细胞的胞质和胞核 ,2 8例乳腺癌组织中PTEN蛋白的表达不同。 14 .2 8%乳腺癌组织无PTEN蛋白的表达 ;2 1.4 %乳腺癌组织PTEN蛋白呈弱阳性 ;6 4 .2 8%乳腺癌组织中有较强的PTEN蛋白表达 ,表达强度与正常乳腺组织无明显差异。此外 ,PTEN蛋白的表达与乳腺癌的临床病理学特点相关 ,PTEN蛋白阴性的乳腺癌 ,2 5 %为雌、孕激素受体阳性 ;而PTEN蛋白表达较强的乳腺癌 ,6 6 .6 7%为雌、孕激素受体阳性 ,且差异有显著性 (P <0 .0 5 )。另外 ,PTEN蛋白阳性的乳腺癌组织 ,腋窝淋巴结转移率为 2 2 .2 % ;而PTEN蛋白阴性的乳腺癌腋窝淋巴结转移率为 75 % ,差异有显著性 (P <0 .0 5 )。结论 乳腺癌组织中存在着抑癌基因PTEN的表达缺失和减弱 ,抑癌基因PTEN的表达异常可能与乳腺癌的发生、发展及预后有关。Objective To investigate the expression and significance of a new tumor suppressor gene PTEN in primary breast carcinoma. Methods Immunohistochemical SP method was used to detect expression of PTEN gene in 28 cases of primary breast carcinoma and 21 cases of normal breast tissues closely adjacent to carcinoma. Immunohistochemical EnVision method was used to detect expression of estrogen receptor and progesterone receptor in these carcinoma tissues. Results It was showed that PTEN gene was expressed in all 21 cases of breast tissues closely adjacent to carcinoma. Expression of PTEN was relatively strong and it was localized in cytoplasm and nuclei of epithelium cells of lobular acini and epithelium cells of ducts. PTEN was expressed in cytoplasm and nuclei of carcinoma cells. Expressions of PTEN in 28 cases of carcinoma were different. 14.28% breast carcinoma tissues were negative of PTEN protein, 21.4% had weak expression of PTEN and 64.28% had strong expression. Tumor suppressor gene PTEN was related to the clinicopathologic features of breast carcinoma. 25% breast carcinoma with negative PTEN protein expressed of ER/PR, while 66.67% with positive PTEN protein expressed ER/PR, and the difference was significant ( P <0.05). In addition, the percentage of lymph node metastasis with primary breast carcinomas positive or negative PTEN was 22.2% or 75% respectively, and the difference was significant ( P <0.05). Conclusion This study suggested that tumor suppressor gene PTEN was abnormally expressed in primary breast carcinoma. PTEN probably plays an important role in the tumorigenesis and development of primary breast carcinoma.
