角膜上皮下羊膜移植术联合绷带式角膜接触镜治疗大泡性角膜病变疗效分析

摘　　要：目的探讨角膜上皮下羊膜移植术联合绷带式接触镜治疗大泡性角膜病变（bullous keratopathy,BK）的临床疗效。方法回顾性分析在我院接受角膜上皮下羊膜移植术联合结膜囊内配戴绷带式角膜接触镜的BK患者28例（28眼）的临床资料。结果所有病例在术后角膜上皮逐渐生长于羊膜上,角膜大泡消失,角膜基质水肿减轻,角膜上皮在5～10天完全修复。20例（71.43%）患者在术后3天内症状消失,无1例羊膜在角膜上皮完全修复前溶解或脱落。术后随访6月,均未复发。13例（46.43%）患者视力改善。结论角膜上皮下羊膜移植术联合绷带式接触镜对于无法恢复视力且眼痛剧烈或经济条件差的BK患者是一种有价值的治疗方法。Objective To evaluate the therapeutic effect of sub-epithelial amniotic membrane transplantation combined with bandage lens for bullous keratopathy（BK）.Methods This was a retrospective clinical study.Twenty-eight patients（28 eyes） with BK undergoing sub-epithelial amniotic membrane transplantation combined with using bandage lens were recruited in this study.Results The corneal epithelium was gradually growing on the amniotic membrane in all the cases.The bullae disappeared and the edema of corneal stroma was alleviated.The corneal epithelium was completely repaired in 5 ～ 10 days in all the patients.There were 20 patients（71.43%） whose symptoms were disappeared within three days after the surgery.No case had amniotic membrane dissolving or falling out before the corneal epithelium repaired completely.During 6 months of follow-up periods,no recurrence was found.The visual acuity of 13 patients（46.43%） was improved.Conclusion The method of sub-epithelial amniotic membrane transplantation combined with bandage lens is an effective treatment for BK patients with poor prognosis of visual acuity or serious symptoms.
