超声刀用于腮腺区域性切除术的临床对照研究

摘　　要：目的比较超声刀和传统方法在腮腺区域性切除中的优缺点。方法选取腮腺良性肿瘤患者60例,按照随机数字表法分为对照组及试验组各30例,对照组采用传统方法行腮腺区域性切除术,试验组采用超声刀手术,全程不用丝线结扎,腮腺残端不缝扎,两组术后均常规给予阿托品肌注防止涎瘘。观察手术时间和术后涎瘘的差异。结果对照组手术时间(109.23±12.59)min,试验组手术时间(89.10±11.27)min,两组比较差异有统计学意义(P<0.05)。试验组术后3天每日引流量(20.41±2.04)ml,明显少于对照组的(29.90±2.54)ml,差异有统计学意义(P<0.05)。试验组2例出现术后涎瘘,对照组5例出现术后涎瘘。结论超声刀可以明显缩短腮腺区域性切除术的手术时间,并且减少术后引流量,操作简单便捷,值得临床推广应用。Objective To compare the disadvantage and advantage between harmonic scalpel and traditional method for parotidectomy.Methods Sixty cases with benign parotid tumor were selected.Of these patients,30 cases were designed as control group treated with traditional operative instruments while another 30 cases were designed as treatment group treated with harmonic scalpel without silk ligature and parotid gland residual transfixion.All the patients were given atropine intramuscularly routinely to avoid salivary gland fistula.The operation time and post-operation salivary fistula were compared.Results There was a significant difference between the control group and the treatment group in terms of operation time[（109.23 ± 12.59） min vs.（89.10 ± 11.27） minutes].The postoperative drainage volume per day in the treatment group（20.41 ± 2.04） ml was significantly lower than that in the control group（29.90 ± 2.54） ml.There were two cases with salivary gland fistula in the treatment group while five cases with salivary gland fistula in the control group.Conclusion Harmonic scalpel can shorten the operation time and reduce postoperative drainage volume.The method is simple and convenient.It is valuable for clinical promotion.
