超声造影辅助穿刺活检对前列腺癌的诊断价值

摘　　要：目的探讨超声造影(CEUS)引导前列腺穿刺活检早期诊断前列腺癌的临床价值。方法对95例可疑前列腺癌患者施行CEUS引导前列腺穿刺活检(6+X),以手术切除标本病理诊断结果为金标准,评估CEUS引导穿刺对前列腺癌的诊断价值。结果 95例患者中最终有27例患者接受前列腺癌根治性切除手术,70例接受经尿道前列腺电切术治疗。与手术标本病理检查结果比较,CEUS引导穿刺法诊断前列腺癌的敏感度为88.0%,特异度为92.9%,阳性预测值为81.5%,阴性预测值为92.8%。结论 CEUS引导前列腺穿刺法在减少穿刺活检针数的同时保持了较好的敏感度和特异度,对前列腺癌早期诊断有一定的临床实用价值。Objective The clinical values of prostate biopsy guided by a contrast-enhanced ultrasound（CEUS） were discussed in this article.Methods Ninety-five patients suspected of prostate cancer received special contrast-enhanced guided prostate biopsies（6 ＋ X）.Pathological outcomes were analyzed and compared between surgical resected samples and prostatic tissues collected by the new method.Results The sensitivity,specificity,positive predictive value and negative predictive value of CEUS guided prostate biopsies was 88.0%,92.9%,81.5% and 92.8%,respectively.Conclusion CEUS guided prostate biopsies has certain clinical value in early diagnosis of prostate cancer because it has advantages of less punctures and excellent diagnostic sensitivity and specificity
