下颌第三磨牙拔除手术的风险评估

摘　　要：目的：本文对阻生下颌第三磨牙（Mandibular Third Molar Impaction，以下简称 M3）的流行病学、病因学、临床应用解剖、影像学检查、并发症及拔除术的手术风险进行文献综述。方法：通过 PubMed、web of science、中国知网对关键字进行搜索，搜索的关键字为 mandibular third molar，lingual nerve injury third molar，inferior alveolar nerve injury third mo-lar，impacted mandibular third molar，时间从1970年到2014年出版。结果：回顾并总结了大量的英文文献，并从多角度深入理解 M3的相关知识，总结了 M3拔除的手术风险。结论：M3拔除的难度与其在下颌骨内的三维解剖位置相关，通过解剖学及影像学的观察，可以有效的评价 M3的拔除风险。另外临床医生还应该考虑到比如年龄、性别、全身状况等，对包括根周膜宽度或者软组织性质的影响，还有患者的心理特点以及临床医师本身的经验水平，只有综合各个因素，才能为患者提供一个有效的治疗方案，提高医师的业务水平。Objective：The present article was performed to review the impacted mandibular third moalr epidemiology,etiology,clinical anatomy,radiologic examination,possible compli-cations and the risk evaluation of removing mandibular third molar.Methods：Literature was acquired through a search of PubMed,web of science and NCBI.The key words used for search were mandibular third molar,lingual nerve injury third molar,inferior alveolar nerve injury third molar,impacted mandibular third molar,and the articles published from 1970 to 2014.Results：Lots of literatures were obtained and reviewed.The relevant knowledge of impacted mandibular third molar was understanded in depth from multiple angles,and summarized the risk of removing man-dibular third molar.Conclusions：The removing dif iculties of M3 related to 3D anatomical position in mandibular,and then the risk of removal could be assessed ef ectively based on anatomy and iconography.In addition,many things needed to consider for doctors,such as the impacts of age,gender,status on periodontal ligament width and properties of soft tissue,psychological characteristic of patients,and personal experience and knowledge.Only by integrating many correlative factors could improve the clinical skil of surgeon and provide an ef ective treatment.
