血清降钙素原和炎性细胞因子的检测与婴幼儿肺炎

摘　　要：目的评价血清降钙素原(PCT),肿瘤坏死因子(TNF-α),淋巴细胞黏附因子-1(ICAM-1)在婴幼儿肺炎早期诊断和治疗的临床意义。方法选取本院收治的48例婴幼儿肺炎患儿作为研究对象,并将其分为轻型组17例,中型组18例,重型组13例;同时选择健康成人22例作为对照组。采用ELISA法检测TNF-α、ICAM-1,速率散射比浊法检测PCT。结果中、重型婴幼儿肺炎组血液中PCT和TNF-α、ICAM-1浓度明显高于对照组和轻型婴幼儿肺炎组;轻型婴幼儿肺炎组PCT高于对照组,但ICAM-1和TNF-α浓度与对照组比较差异无统计学意义。不同病情婴幼儿肺炎血液中PCT和TNF-α、ICAM-1与病情呈正相关。结论联合PCT与TNF-α、ICAM-1三者测定能够更早、准确、灵敏地诊断婴幼儿肺炎。Objective To evaluate the clinical significance of early diagnosis and treatment of serum procalcitonin（PCT）, tumor necrosis factor alpha（TNF-α）, lymphocyte adhesion molecule-1 （ICAM-1） in infantile pneumonia. Methods ELISA assay for detection of TNF-α, ICAM-1, PCT was measured with rate nephelometry. Results The blood PCT and TNF- α, ICAM-1 concentration of medium and heavy infant pneumonia group was higher than that of the control group and the light infant pneumonia group;PCT of light infant pneumonia group was higher than control group, but there was no significant in ICAM-1 and TNF-αconcentrations compared with control group. The severity of pneumonia in blood PCT and positive TNF-α, ICAM-1 and disease related. Conclusion Combination of PCT, TNF-αand ICAM-1 is a good way for accurate and early diagnosis of infantile pneumonia.
