双相情感障碍抑郁发作与单相抑郁症患者执行功能损害的对照研究

摘　　要：目的探讨双相情感障碍抑郁发作患者与单相抑郁患者执行功能损害的差异。方法收集符合美国精神障碍诊断与统计手册第4版诊断标准的双相情感障碍抑郁发作患者30例和抑郁症患者30例。采用威斯康星卡片分类测验(Wisconsin Card Sorting Test,WCST)评定执行功能,韦氏成人智力量表(Wechsler Adult Intelligence Scale Revised,WAIS-R)评定2组智商,汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)17项评定抑郁严重程度。结果双相情感障碍组总应答数(Ra)、错误应答数(Re)及持续性错误数(Rpe)评分均差于单相抑郁组,差异有统计学意义(P<0.05)。结论抑郁症状严重程度相同时,双相情感障碍对执行功能的损害比单相抑郁对患者的影响更加严重。Objective To explore the characteristic of executive function impairment in unipolar and bipolar depressive disorders.Methods We selected respectively 30patients meeting with diagnosis of unipolar and bipolar depressive disorders and statistical manual of mental disorders-fourth edition（DSM-IV）.The people in each group should be given clinical assessment,including Wisconsin Card Sorting Test（WCST）,Hamilton Depression Scale（HAMD）,and Wechsler Adult Intelligence Scale Revised（WAIS-R）.Results The scores of Response administered（Ra）,Response errors（Re）and Response perseverative errors（Rpe）is lower in unipolar depressive disorders,and there were significantly different（P〈0.05）.Conclusion With the same degree of depression,executive function impairment in bipolar depressive disorders is more serious than that of unipolar depressive patients.
