高压氧联合康复训练和针刺综合治疗脑卒中后早期吞咽障碍的疗效

摘　　要：目的观察联合采用高压氧(HBO)、康复训练及针刺综合治疗脑卒中后早期吞咽功能障碍的疗效。方法 80例脑卒中早期伴吞咽障碍患者,随机分为高压氧组(HBO组)、针刺组、康复训练组及联合治疗组。各组患者均给予常规药物治疗,在此基础上HBO组同时辅以高压氧干预,针刺组辅以针刺天突等穴位,康复训练组辅以吞咽功能训练及德国菲兹曼Vocastim-master吞咽语言治疗仪,联合治疗组则给予HBO、针刺、吞咽功能训练及德国菲兹曼Vocastim-master吞咽语言治疗仪联合治疗。分别于治疗前及治疗20 d后进行洼田饮水试验和吞咽X线电视透视检查(video fluoroscopy swallowing study,VFSS)。结果各组患者洼田饮水试验和VFSS评分均较治疗前明显改善(P<0.05或P<0.01);进一步研究发现,各组患者吞咽功能疗效存在组间差异(P<0.05):联合治疗组>康复训练组>针刺组>HBO组。结论 HBO联合康复训练和针刺综合治疗脑卒中后早期吞咽障碍具有协同效应,可进一步提高临床疗效,促进患者吞咽功能恢复。Objective To observe the curative effect of hyperbaric oxygen （HBO）with rehabilitation training and acupuncture to treat post-stroke dysphagia in early stage.Methods 80 cases of post-stroke dysphagia in early stage were random ly divided into HBOgroup, acupuncture group ,rehabilitation training group ,and com bined treatm ent group .E ach group w as given conventional m edical treatm ent,and HBOgroup were given hyperbaric oxygen intervention ,acupuncture group were given acupuncture at Tiantu acupoint,rehabilitation training group were given sw allowing function training and G erm an V ocastim-m aster deglutition language therapeutic apparatus,and com bined treat-m ent group were given hyperbaric oxygen （HBO）,acupuncture,sw allowing function training and G erm an V ocastim-m aster deglutition lan-guage therapeutic apparatus.K ubota drinking w ater test and X-ray video fluoroscopy sw allowing study （V FSS ）were conducted before treat-m ent and 20 days after treatm ent.Results K ubota drinking w ater test and V FSS scores in each group clearly im proved after treatm ent com-pared to before treatm ent（P〈0.05 or P〈0.01）.Further study show ed that curative effect of sw allowing function in each group had differ-ences （P〈0 .05 ）in the follow ing order of com bined treatm ent group〉rehabilitation training group〉acupuncture group〉HBOgroup . Conclusion H yperbaric oxygen （HBO）with rehabilitation training and acupuncture to treat post-stroke dysphagia in early stage has synergis-tic effect,w hich can further im prove clinical effects and accelerate sw allowing function recovery.
