腹腔置管联合鸦胆子油乳和白介素-2治疗老年恶性腹腔积液的效果

摘　　要：目的观察腹腔穿刺并单腔中心静脉导管置管引流联合腹腔内使用鸦胆子油乳和白介素-2（IL-2）治疗老年恶性腹腔积液的效果和不良反应。方法回顾性分析2007年1月～2010年10月华中科技大学同济医学院附属荆州医院38例老年恶性腹腔积液患者的临床资料,所有患者均于腹腔穿刺置管引流后腹腔内使用药物,将其随机分为两组,观察组19例,腹腔内使用鸦胆子油乳和IL-2;对照组19例,腹腔内单独使用IL-2。观察两组患者的临床疗效和不良反应。结果观察组总有效率为78.9%,明显高于对照组（47.4%）,差异有统计学意义（P〈0.05）。两组患者不良反应均较少。结论腹腔穿刺并单腔中心静脉导管置管引流联合腹腔内使用鸦胆子油乳和IL-2是老年恶性腹腔积液的一种有效、安全的治疗模式。Objective To evaluate the efficacy and safety of abdominal indwelling catheter combined with Brucea Javanice Oil Emulsion and IL-2 in malignant peritoneal effusion. Methods 38 cases with malignant peritoneal effusion confirmed by pathology from January 2007 to October 2010 in Jingzhou Hospital Affiliated to Tongji Medical College,Huazhong University of Science and Technology were analyzed retrospectively. They were all placed drain by abdominal centesis and divided into two groups, 19 cases of the observation group and 19 cases of the control group. observation group were given the Brucea Javanice Oil Emulsion and IL-2 injected into abdominal cavity and the control group was only given the IL-2 injected into abdominal cavity. Results The total effective rate of observation group was78.9%, which was obviously higher than that of control group（47.4%）, the difference was statistically significant（P〈0.05）, less adverse reaction in both groups. Conclusion The therapeutic modality, Brucea Javanice Oil Emulsion and IL-2 injected into abdominal cavity after abdominal indwelling catheter is effective and safe in treating malignant peritoneal effusion.
