着丝粒蛋白-H在胃癌组织的表达及其与临床、病理及预后的关系

摘　　要：目的 检测胃癌组织中着丝粒蛋白-H（CENP-H）的表达,探讨CENP-H在胃癌组织中表达的临床病理意义。方法 应用实时定量聚合酶链反应（Real-time PCR）和Western blot检测9对新鲜胃癌组织和癌旁非肿瘤黏膜组织CENP-H的mRNA转录和蛋白表达,免疫组织化学染色（IHC）检测166例胃癌组织及对照非肿瘤胃黏膜CENP-H的蛋白表达,结合临床资料分析其与临床病理特征之间的关系。结果 新鲜组织中,胃癌组织CENP-H的蛋白水平（P〈0.05）及mRNA（P〈0.01）均明显高于胃黏膜组织。IHC显示,CENP-H在胃癌组织阳性率为51.2%,而在非肿瘤黏膜中染色阴性。CENP-H表达与肿瘤直径（P〈0.05）、浸润深度（P〈0.01）、淋巴结转移（P〈0.01）、TNM分期（P〈0.01）、预后（P〈0.01）明显相关,Log-rank检验表明CENP-H高表达患者5年生存率明显降低（P〈0.01）,Cox回归多因素分析显示,CENE-H（P〈0.05）、淋巴结转移（P〈0.01）、TNM分期（P〈0.01）是胃癌的独立预后因素。结论 CENP-H在胃癌组织中表达阳性率高,可能是一个促癌基因,在胃癌中,CENP-H高表达提示患者预后不良。Objective To explore the expression of centromere protein H （CENP-H） and its corre- lation with clinicopathological features of gastric cancer （GC）. Methods The expression level of CENP-H in 9 caes of gastric cancer tissue and corresponding normal mucosal tissue was detected by real-time quantitative polymerase chain reaction （ Real-time PCR） and Western blotting. CENP-H expression levels in specimens from 166 patients with GC and nonneoplastic mucosal tissue were determined by immunohistochemical staining. The correlations between CENP-H with clinicopathologic features and prognosis was analyzed. Results It was clearly showed that the mRNA and protein expression of CENP-H was elevated in tumor tissue samples as compared with that in nonneoplastic mucosal tissue （ P 〈 0.05 ）. The CENP-H expression was positive in 85 of 166 cases of GC, and negative in all nonneoplastic mucosal tissues, showing a significant correlation with tumour size （P 〈 0.05 ）, depth of infiltration （ P 〈 0. 01 ）, lymph node metastasis （ P 〈 0.01 ） and TNM staging （ P 〈 0. 01 ） of GC, as well as clinical prognosis （P 〈 0. 01 ）. Log-rank test indicated that 5-year survival rate of patients with high CENP-H expression was significantly lower than that of patients with low CENP-H expression. Muhivariate Cox regression analysis confirmed that CENP-H （ P 〈 0. 05 ）, lymph node metastasis （ P 〈 0.01 ） and TNM staging （P 〈 0. 01 ） were independent prognostic predictors for patients with GC. Conclusion Our data suggested that CENP-H might play a role as a cancer gene and that its hizh expression indicated a Door oroanosis in GC.
