湖南省3451例宫颈人乳头状瘤病毒感染状况分析

摘　　要：目的了解湖南省3451例宫颈人乳头状瘤病毒（HPV）感染状况和基因亚型分布。方法采用流式荧光杂交技术对湖南地区3451例女性进行HPV分型检测，并对HPVDNA亚型、感染率和年龄分布进行分析。结果3451例女性中检出HPV阳性917例，阳性率为26．57％。阳性感染者中，单一型感染最多，为665例，占72．52％，其中高危单一型感染者539例，占58．78％，低危单一型感染者126例，占13．74％，混合型感染者252例，占27．48％，主要为双重感染，占19．74％。19个与宫颈癌密切相关的HPV高危亚型阳性率为22．83％，7个低危亚型HPV阳性率为6．69％。感染排前8位的亚型分别是HPV52（23．66％）、HPV16（16．58％）、HPV6（11．34％）、HPV58（11．01％）、HPV39（7．96％）、HPV33（6．43％）、HPV68（6．22％）、HPV18（5．78％）。在各年龄组中，〈25岁组感染率最高，为34．1％，〈25岁组与25～44岁组感染率差异有统计学意义（P〈0．05）；45～54岁组感染率与25～44岁组差异有统计学意义（P〈0．05）。结论湖南地区女性HPV感染以高危型感染为主，HPV52、16、6、58亚型是最主要的感染亚型，进行HPVDNA检测和分型对宫颈病变的防治具有重要意义。Objective The aims of this study were to know the infection status and genotype distribution about the cervical human papillomavirus（HPV） of 3 451 cases in Hunan Province. Methods A total of 3 451 women infected with the human papillomavirus in Hunan province were detected by the flow-fluorescence hybridization./;The genotype,infection rate and age distribution were analyzed. Results Nine hundred and seventeen cases in 3 451 women had the positive results with HPV,and the positive rate was 26.57%. In the 917 cases with HPV,the main type was single type infection with the number as 665,and the percentage was 72.52%. Five hundred and thirty-nine of the 665 cases with single HPV genotype infection were infected with high-risk types,and the percentage was 58.78%. One hundred and twenty-six cases were infected with low-risk types,and the percentage was 13.74%. Two hundred and fifty-two cases were infected with mixed types, and the main infection type was dual type infection. The percentage of mixed type infection was 19.74%. The total positive rate of 19 cervical cancer high-risk types was 22.83%, and the total positive rate of 7 cervical cancer low-risk types was 6.69%. The top eight HPV infection genotypes were HPV52 （23. 66%）, HPV16 （16. 58%）, HPV6 （11. 34%）, HPV58 （11. 01%）, HPV39 （7. 96%）, HPV33 （6. 43%）, HPV68（6.22%） ,and HPV18（5.78%）. Among the different age groups,the less than 25 years group had the highest infection rate with the percentage as 34.1%. The infection rates had significant difference between the group less than 25 years and the group from 25 years to 44 years. The infection rates had significant difference between the group from 45 years to 54 years and the group from 25 years to 44 years. Conclusion The main HPV infection types in Hunan province are high-risk type. The main HPV types are HPV52,16,6,58. The detection and genotyping of HPV DNA are important for the prevention of cervical cancer.
