骨良性纤维组织细胞瘤的临床及影像学表现

摘　　要：目的探讨骨良性纤维组织细胞瘤的影像学特征,提高对该病的影像学诊断水平及鉴别诊断能力。方法收集5例骨良性纤维组织细胞瘤患者的临床及影像学资料,结合相关文献复习进行回顾性分析。结果 4例为单发病变,1例上下相邻两个病灶。X线平片表现为类圆形骨质破坏区,呈"皂泡状"或"蜂房状",边界清楚;CT表现为病灶周边环状硬化边,其内软组织密度,未见完整骨性分隔;磁共振成像(MRI)平扫T2WI低信号为主,T1WI及质子密度加脂肪抑制见病灶中央低信号,中间呈花环状T1WI稍低信号、质子密度加脂肪抑制高信号,外周带均呈环状低信号;增强扫描病变中间花环状强化呈高信号,病变中央及外周带未见强化。结论骨良性纤维组织细胞瘤影像学表现具有一定特征性,认识其表现有助于对该病的诊断和鉴别诊断。Objective To evaluate the imaging characteristics of bone benign histiocytoma and promote imaging diagnosing the level and differential diagnosing ability. Methods The clinical and imaging data of 5 cases bone benign histiocytomas were received and retrospectively analyzed. Relative literatures were reviewed. Results The locations of 5 patients were respectively femoral upside between tuberosities,metaphysis of femoral downside,tibial upside and tibial downside. The manifestations of X ray were resemble circular areas of destroyed bones that appeared foamy or cellular shapes. The areas of destroyed bones had clear margins and intact cirrhosis loop in 4 patients,half- baked cirrhosis loop in 1 patients. All the areas of destroyed bones did not have bone- membrane reaction and parenchyma bump,but had parenchyma swelling in 1 traumatic patient. Single focus in 4 patients and 2 border upon focus in 1 patient were seen. The CT manifestations of destroyed bones had intact cirrhosis loop in 3 patients. Soft tissue densities were seen inside the loops and its compartmentations of bone were discontinuous. MRI were examined in 3patients,compared with round signal of marrow,focus manifested low signals in T2WI. In T1WI and PDWI ＋ FS centers of focus manifested low signals around which were irregular mild low signals. Circular low signals in outside focus were seen. The irregular low- grade low signals were enhanced after contrast- agent were injected and remain parts of focus weren＇t enhanced. Conclusion The imaging manifestations of bone benign histiocytomas had some characteristics. Knowing the characters can be used for diagnosing and differential diagnosing to bone benign histiocytomas.
