长春瑞滨、顺铂、氟尿嘧啶三联方案与顺铂联合氟尿嘧啶方案治疗老年晚期食管癌的临床效果对比研究

摘　　要：目的比较长春瑞滨、顺铂、氟尿嘧啶三联治疗方案与顺铂联合氟尿嘧啶治疗方案应用于老年晚期食管癌患者的临床疗效。方法选择2010年6月至2013年6月我院收治的老年晚期食管癌患者68例,随机分为A组和B组,每组各34例。A组采用顺铂联合氟尿嘧啶治疗方案,B组采用长春瑞滨、顺铂、氟尿嘧啶三联方案。每3周为1个周期,连续使用2周后评定患者的临床效果。对两组患者治疗后的近期疗效及毒副反应进行比较。结果 B组患者的有效率(61.8%)高于A组(38.2%),差异有统计学意义(P<0.05)。B组患者的中位生存期为10.2个月,A组患者的中位生存期为8.7个月,差异有统计学意义(P<0.05)。B组患者的骨髓抑制及消化道反应与A组患者比较,差异无统计学意义(P>0.05)。结论长春瑞滨、顺铂、氟尿嘧啶三联方案治疗老年晚期食管癌,可提高疗效及患者的生活质量,值得临床推广与应用。Objective To study the clinical effect of vinorelbine,cisplatin combined with 5-fluorouracl and cisplatin combined with 5-fluorouracl in the treatment of elderly advanced esophageal carcinoma.Methods 68 cases of patients with advanced esophageal carcinoma in our hospital from June 2010 to June 2013 were randomly divided into group A and group B,34 cases in each group.Group A was given cisplatin combined with 5-fluorouracl,and group B was given vinorelbine,cisplatin combined with 5-fluorouracl.Every three weeks for a period,to assess the clinical effects of the two gwups after two weeks continuous use in patients.The short time curative effect and toxicity reaction were compared between the two groups after treatment.Results The effective rate in group B（61.8%） was higher than that in group A（38.2%）,there was significant difference between the two groups（P〈0.05）.The mediansurvival time of patients in group B was 10.2 months,and that in group A was 8.7 months,there was significant difference between the two groups（P〈0.05）.There were no significant difference of the incidence of neutropenia and digestive tract reaction between the two groups（P〉0.05）.Conclusion Vinorelbine,cisplatin combined with 5-fluorouracl in treating elderly advanced esophageal carcinoma can improve the curative effect and quality of life.It deserves to be further popularized clinically.
