摘　　要：目的了解医院住院不合理用药医嘱的发生原因并进行分析。方法随机抽取医院2012年6月至11月出院病历180份,对其中的不合理用药医嘱进行统计和分析。结果 180份病历中,不合理病历39份,不合理用药医嘱56例次。不合理情况主要涉及适应证、遴选药品、药品剂型或给药途径、用法用量等。结论医院应重视医嘱点评工作,提高合理用药水平,保证患者安全、有效、经济用药。Objective To understand the situation of irrational medication orders in the inpatients of our hospital and to analyze. Methods 180 discharged inpatient medical records were randomly extracted in our hospital from June to November 2012,in which the irrational medication orders were analyzed statistically. Results In 180 medical records, 39 medical records were irrational and 56 case- times were irrational medication orders. The irrational situation mainly involved in the indications, selection of drugs, drug dosage forms or drug administration route,usage, dosage, etc. Conclusion Hospital should pay great attention to the comments work of medication orders for improving the level of rational drug use and ensuring safe,effective and economic medication of patients.
