前房注入不同物质在小切口白内障囊外摘除患者中应用价值的对比研究

摘　　要：目的探讨小切口白内障囊外摘除术毕前房注入滤过空气或平衡盐溶液的临床应用价值。方法将50例64眼年龄相关性白内障与代谢性白内障患者随机分成为观察组和对照组,均行小切口白内障囊外摘除术,术毕前房注入滤过空气的为观察组25例(32眼),注入平衡盐溶液的为对照组25例(32眼),分析两组患者术后裸眼视力、前房深度、眼压与角膜内皮细胞密度。结果 (1)术后3、5 d,观察组的裸眼视力显著高于对照组的(P<0.05),观察组的裸眼视力均集中在≥0.6。(2)术后1、3、5 d,观察组的前房深度显著高于对照组的(P<0.05)。(3)术后1、3、5 d,观察组的眼压显著高于对照组的(P<0.05)。(4)术后1 d时观察组的角膜内皮细胞密度显著高于对照组的(P<0.05)。结论行小切口白内障囊外摘除术毕前房注入滤过空气,有利于前房形成与维持、视力的恢复、前房深度的加深与维持眼压的正常水平,角膜内皮细胞损伤小,减小了并发症的发生率。Objective To investigate the clinical value of filtered air or balanced salt solution in mini-incision extracapsular cataract extraction. Methods Fifty cases were selected as observation group（25 cases /32 eyes） whose anterior chamber were injected with filtered air,and control group（25 cases /32 eyes） whose anterior chamber were injected with balanced salt solution. The uncorrected visual acuity,anterior chamber depth,intraocular pressure and corneal endothelial cell density of both groups were compared. Results The uncorrected visual acuity of observation group was significant higher than that of control group（P〈0. 05） after postoperative 3 d and 5d,and observation group＇s uncorrected visual acuities were equal or greater than 0. 6 The anterior chamber depth of observation group was significantly higher than that of control group after postoperative 1 d,3 d and 5 d（P〈0. 05）. The intraocular pressure of observation group was significantly higher than that of control group after postoperative 1d,3d and 5 d（P〈0. 05）. The corneal endothelial cell density of observation group was significantly higher than that of control group（P〈0. 05） after postoperative 1 d. Conclusions The anterior chamber injected with filtered air in mini-incision extracapsular cataract extraction is advantageous to the development of anterior chamber,the recovery of visual acuity,anterior chamber depth,the normal level of intraocular pressure and can reduce the injury of corneal endothelial cell and the incidence of complications.
