摘  要:目的探讨乙型肝炎病毒(HBV)感染与弥漫大B细胞淋巴瘤(DLBCL)预后的相关性。方法选取乙型肝炎表面抗原(HBsAg)阳性和HBsAg阴性的初治DLBCL患者各60例,均给予化疗,比较分析两组患者化疗期间的肝功能以及随访2年后的无进展生存时间(PFS)和总生存时间(OS)。结果 HBsAg阳性组和HBsAg阴性组患者化疗期间的肝功能损害发生率分别为66.0%和40.0%,差异有统计学意义(P<0.05)。HBsAg阳性组患者的PFS为(16.1±3.2)个月,PFS率为70.0%,OS率为85.0%;HBsAg阴性组患者的PFS为(17.3±4.1)个月,PFS率为73.0%,OS率为88.0%,两组差异无统计学意义(P>0.05)。结论 HBV感染与DLBCL患者的预后无关,但合并HBV感染会增加DLBCL患者在化疗期间肝功能损害的发生率。Objective To investigate the correlation between prognosis of patients with diffuse large B-cell lymphoma(DLBCL) and hepatitis B virus(HBV) infection. Methods 60 HBsAg-positive patients and 60 HBsAg-negative patients with DLBCL were selected, all of the patients were treated by chemothera- py. The liver function during the chemotherapy, the progression-free survival time (PFS) and overall survival time(OS) in the followed 2 years were observed and compared between the two groups. Results The liver function damage rates during chemotherapy of HBsAg-positive group and HBsAg-negative group were 66.0% and 40. 0% ,respectively(P 〈 0. 05 ). The 2-year PFS in the HBsAg-positive group was ( 16. 1 + 3.2) months,with a PFS rate of 70. 0% and an OS rate of 85.0%. In the HBsAg-negative group the PFS was ( 17.3 ~ 4. 1 ) months,the PFS rate was 73.0% and the OS rate was 88.0% ,which were consistent with the HBsAg-postive group(P 〉 0. 05). Conclusion The HBV infection had no significant impaction on clinical features and prognosis in patients with DLBCL, but it can increase the risk of liver damage during the chemotherapy.
