摘 要:目的利用微创枕大池二次注血方法及交叉前池注血法建立蛛网膜下腔出^(sari)后脑血管痉挛(cvs)大鼠模型,对比2种方法利弊及效果分析。方法60只SD大鼠按随机数字表法分为对照组、枕大池二次注血法假手术组、交叉前池注血法假手术组、枕大池二次注血法SAH组、交叉前池注血法SAH组,每组各12只。分别行神经功能评分,标本大体观察,大脑中动脉及基底动脉显微观察,检测脑脊液降钙素基因相关肽(CGRP)含量及在基底动脉血管壁上的表达。结果枕大池二次注血法SAH组与交叉前池注血法SAH组相比神经功能缺失更为显著;二次注血法获得的血管痉挛程度较交叉前池注血法更为严重,差异有统计学意义(P〈0.05);SAH组脑脊液CGRP含量及基底动脉内膜CGRP的表达较正常及假手术组相比明显减少,差异有统计学意义(p<0.05)。结论交叉前池注血法适合前循环动脉瘤造成SAIl的病理生理机制的研究,微创枕大池二次注血法适合后循环椎基底动脉或后交通动脉瘤造成SAH的病理生理机制的研究。Objective Subarachnoid hemorrhage (SAH) could be induced in rats by minimally-invasive autologous blood injection into cistema magna (ACM) and prechiasmatic cistern (APC), respectively. These two models of cerebral vasospasm (CVS) are compared to provide experimental basis. Methods Sixty SD rats weighting 250 to 300 g were randomly assigned to 5 groups (n=12): control group, sham-operated groups (injecting normal saline via ACM and APC) and SAH groups (injecting autologous blood via ACM and APC). Neurological outcomes were evaluated with Neurologic Scale System; the basilar cistern was observed; the inner diameters of the blood vessel wall of the middle cerebral artery (MCA) and basal artery (BA) were measured under microscope; the concentration of calcitonin gene-related peptide (CGRP) in the cerebrospinal fluid was tested and its expression in the basal arterial wall was detected by immunofluorescence method. Results ACM models of SAH group showed more serious neurological dysfunction than APC models. ACM models developed more severe CVS than APC models of SAH group (P〈0.05). The contcentration of CGRP in cerebrospinal fluid and the CGRP expression in the basal artery intima in SAH groups were less than those in the control group and sham-operated groups (P〈0.05). Conclusion APC models are suitable for studying the pathophysiological mechanism of intracranial anterior circulation aneurysmal SAH while ACM models are eligible for studying the pathophysiological mechanism of intracranial posterior circulation aneurvsmal SAH.
