乳腺癌患者新辅助化疗前后IGF-1的改变

摘　　要：目的探讨与分析乳腺癌患者胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)在新辅助化疗前后的改变情况。方法将2010年10月到2013年10月于我院接受新辅助化疗的40例乳腺癌患者作为研究对象,所有患者均采用新辅助化疗,持续4周期左右,并在间隔2周后施行乳腺改良根治手术,测定化疗前后患者血清中IGF-1表达水平。结果施行新辅助化疗前40例患者的IGF-1表达水平为(53.09±3.99)ng/ml,施行新辅助化疗3个周期后为(33.67±3.73)ng/ml,前后比较差异显著(P<0.05)。年龄、TMN分期与IGF-1的浓度有关,无论化疗前后,年龄<45岁者的IGF-1浓度高于≥45岁者,TMN分期Ⅲ期的IGF-1浓度高于Ⅱ期者。同时,化疗后IGF-1浓度均显著下降;化疗1、3、4个周期后,IGF-1浓度逐渐下降,且小于<45岁者更为明显。结论乳腺癌患者血清IGF-1浓度与肿瘤的进展有关,可能有评估新辅助化疗疗效与预后的潜能。Objective To investigate the changes of insulin-like growth factor-1 （IGF-1） in patients with breast cancer before and after neoadjuvant chemotherapy. Methods From October 2010 to October 2013 in our hospital, there were 40 cases of breast cancer treated with neoadjuvant chemotherapy enrolled as the research ob- jects. New adjuvant chemotherapy was used for about 4 cycles, and after the interval of 2 weeks, breast modified radical surgery was undergone. And before and after chemotherapy, the serum IGF-1 expression level was exam- ined. Results The expression level of IGF 1 before neoadjuvant chemotherapy was （53.09 ± 3.99） ng/ml, which was much higher than （33.67 ± 3.73 ） ng/ml after chemotherapy （ P 〈 0.05 ）. And age and TMN staging was related to the concentration of IGF-1. Both before and after chemotherapy, the IGF-1 concentrations in pa- tients 〈 45 years was higher than ≥45 years . IGF-I concentration of TMN stage Ⅲ was also higher than stage Ⅱ. At the same time, the concentration of IGF-1 after chemotherapy was significantly decreased. After 1,3,4 cycles of chemotherapy, IGF-1 concentration gradually decreases, which was more obvious in the patients 〈 45 years old. Conclusion Serum IGF-1 concentration is associated with the progress of breast cancer, and may be a po- tential prognosis factor for evaluating curative effect of new adjuvant chemotherapy.
