摘  要:目的:研究口腔常用修复材料在口腔颌面锥形束CT和螺旋CT中的二维图像伪影,为临床口腔修复材料选择和口腔放射检查提供参考依据。方法:采用猪头颅(6个月龄,长白猪),模拟人在体口腔环境,使用氧化锆、纯钛、镍铬、钴铬、银钯、金钯6种修复材料,在同牙位制作全冠,分别戴入右下第1、2、3前磨牙,分别按常规颌骨CT条件使用口腔颌面锥形束CT(90ekV,0.4mm)和16排螺旋CT(130ekV,1mm)扫描,测量伪影的尺寸、面积和形态。结果:6种材料在两种CT中都会产生以冠为中心向四周散射的伪影,同种材料在口腔颌面锥形束CT和螺旋CT中产生的伪影形态相似,但是尺寸和面积前者小于后者,差异具有统计学意义(P<0.05)。材料的密度越高,投照的强度越大,伪影的面积越大,沿投照方向的条索状伪影越大,差异具有统计学意义(P<0.05)。不同材料产生的高亮度伪影和条索状伪影排列顺序为纯钛<氧化锆<镍铬<钴铬<金钯<银钯,差异具有统计学意义(P<0.05)。结论:当患者口腔内有高密度修复材料存在时,应首选低投照强度的设备检查,固定修复时应首选密度较低的材料。Objective: To compare the image artifacts of common dental restorative materials between oral and maxillofacial cone beam CT and 16-slice spiral CT. Method.. Pig skulls ( six months old , Landrace ) are used to simulate human in vivo oral environment. Six dental restorative materials (zirconia, titanium , nickel, chromium , cobalt, chromium, silver palladium , gold palladium) were used to make full crown prosthesis in the same tooth bit of the bottom right of the first , second and third premolars. The size, area and shape of image artifacts are measured in conventional mandibular CT conditions by using oral and maxillofacial cone-beam CT (90 ekV, 0.4mm) and 16- slice spiral CT (130ekV, 1mm). Results:Six dental restorative materials will have the surrounding scattering artifacts from the crown as the center in both CT. The image artifacts of the same material have similar morphology in oral and maxillofacial cone beam and spiral CT, but the size and area of the former is smaller than the latter. The difference is considered statistically significant (P〈0.05). The higher the density of the material, the greater the intensity of projection will generate the larger the area of artifacts and the greater the projection direction along the streak artifacts. The difference is considered statistically significant (P〈0.05). Different materials produced high brightness artifacts and streak artifacts in the order of titanium 〈 zirconia 〈 nickel-chromium 〈 cobalt chromium 〈 Au-Pd 〈 silver palladium. The difference is considered statistically significant (P〈0.05). Conclusion: When a patient's oral has high-density dental restorative materials, the low intensity projection device should be used. Low-density material should be used in fixed prosthesis.
